The Rural Clinical School Tracking Project: More IS better – Confirming factors that influence early career entry into the rural medical workforce

Practice points § Australian rural clinical schools were established in 2002 to increase the rural medical workforce. § Cross-sectional studies are beginning to show a positive impact on interest in pursuing rural practice after graduation. § Only longitudinal studies will determine what factors are predictive of medical graduates eventually choosing a rural workplace location and confirm the efficacy of this impact. § Our study found the most important factor regarding eventual rural workplace choice and interest in a future rural medicine career was spending two as opposed to one clinical year at a rural clinical school. § In the face of increasing medical student intake and the consequent strain on resources and teaching capacity, the importance of the length and quality of rural clinical education on future rural workforce potential should not be compromised.

 

o 배경

¡ 호주 정부의 Rural Clinical Schools initiative는 2000년~2001년 사이에 도입되었으며, 의과대학생들이 호주 전역에 걸쳐서 병원, 일반진료외과(general practice surgeries), 지역의료센터 등에서 실습 기회를 가질 수 있도록 기회를 제공하고 있음. 이 프로그램의 일차적 목표는 농촌 의료인력의 모집과 유지(recruitment and retention)임. 

¡ RCS는 현재 호주의 14개 의과대학에 설립되어 있으며, 이러한 농촌 지역 의사 부족 문제에 대한 RCS의 효과는 장기적으로 어떻게 진행되느냐에 따라 달려 있을 것임. 단기적인 성과(인턴 선택, 농촌 진료에 대한 태도 등)에 대한 단면 연구는 긍정적인 결과를 보여주고 있음. 

¡ Queensland 대학의 RCS는 의과대학의 4년제 MBBS 프로그램의 한 부분으로서, 3학년과 4학년 기간 동안 학생은 네 개의 임상 수련 병원이 있는 UQRCS에서 수련을 받을 수 있음. UQRCS의 첫 코호트는 현재 5년차 의료인력임. 

 

¡ 우리의 종단 연구의 목적은 두 가지로, 한 가지는 후향적 코호트 연구를 통해서 졸업 후 RCS 동문들의 의료참여 패턴(workforce participation patterns)를 알아보는 것이며, 두 번째 단계로서는 전향적 종단 연구를 통해서 UQRCS 동문이 어떤 진로 경로를 따르는지 보는 것임.

¡ 진로 선택은 다양한 개인적 차이에 의해서 영향을 받는데, 크게 네 가지 차이가 다른 문헌을 통해서 보고된 바 있음(personality traits, career interests, values and attachments styles). 본 연구에서 보고자 하는 추적 관찰과 함께 이러한 차이를 함께 연구한다면 더 심도 있는 이해를 할 수 있을 것임.

 

o 방법

¡ 본 연구에서는 2002년과 2006년 사이에 UQRCS의 3학년과 4학년을 보냈던 모든 졸업생을 대상으로 후향적 웹 기반 설문조사를 실시하였음. 기본 정보 외에도 현재 근무지, 전공 선택 등을 평가하였음.

 

o 결과

¡ UQRCS에서 2년(3학년, 4학년)을 보낸 동문은 농촌 의사의 진로를 선택한 경우가 유의미하게 높았으며, 현재 비도심(non-urban)지역에서 근무하고 있는 동문은 UQRCS에서 2년을 보냈고, 농촌에 남고자 하는 의사가 유의미하게 높은 것을 확인하였음. UQRCS에서 보낸 전체 시간이 농촌 의사로서의 진로에 대한 관심도에 가장 크게 기여하는 요인이었음. 일반의(General practice)와 응급의(emergency medicine)을 주로 선호하였음.

 

o 결론

¡ 본 연구결과로부터 농촌 임상 경험을 더 오래 할수록 결과적으로 미래에 농촌에서 근무하게 되고, 더 높은 관심을 가지게 됨을 확인하였음.

Abstract

INTRODUCTION:

This longitudinal tracking project by the University of Queensland Rural Clinical School (UQRCS) provides data on the early careerpathways of alumni since its inception in 2002. Specifically we report on; (1) perceptions of the UQRCS regarding its influence on current careerchoice, (2) current speciality choices, and (3) factors that influenced these choices.

METHODS:

A retrospective web-based survey of all graduates who undertook clinical Year 3, Year 4 or Years 3 & 4 at UQRCS from 2002 to 2006. Associations with demographics, Year(s) at UQRCS, current workplace and speciality choice were assessed.

RESULTS:

Response rate was 69%, (N=180). Alumni who spent two years (Years 3 & 4) at UQRCS reported significantly higher degrees of encouragement to pursue a rural career. Alumni currently working in a non-urban location had spent two years at UQRCS and were significantly higher in desire to remain rural. 'Time spent at the UQRCS' contributed most to interest in a rural career. General practice and emergency medicine were most preferred specialities.

CONCLUSIONS:

Findings confirm that a longer rural clinical experience is more effective in eventual choice of workplace location and future interest in a rural career. This corroborates the positive impact of Australian rural clinical schools toward rural medical workforce improvement.
