북한이탈의사 교육: 서울의료원에서의 경험을 중심으로

서 론

북한의 보건의료체계는 무상치료제, 의사담당구역제, 예방의학을 강조하는 보건학적으로 우수한 개념을 갖고 있음에도 지속적인경제적 악화, 자연재해로 인한 의료시스템의 붕괴로 약제, 진료장비 등의 인프라가 지속적으로 공급되지 못하고 병의원에서 의료진이 진료할 수 없는 상황에 이르렀다(이성봉, 2009). 기초적인 의료자원의 부족과 느슨해지는 국가통제, 배급시스템의 약화로 의료체계는 정성운동기에서 장마당, 암시장체계로의 전환이 이루어진 것으로 보인다(최영인 외, 2006). 의사나 환자들이 약을 구하여 치료할 수 없는 상황, 환자가 의료진을 배제한 채 임의로 암시장에서 매약하고 자가 치료하는 행태로 인해 의료체계와 의학교육은 파행을겪고 있는 것으로 판단된다. 의사들은 어려운 환경 속에서 교육을받고 진료에 임하여 오고 있었으며 생존과 탈북의 과정에서 의사로서 일을 중단하는 경험을 하게 된다. 탈북에 성공하여 남한에 온 북한이탈주민의사들은 북한과 다른 의료사회문화 환경, 영어 중심의다른 의학용어의 사용, 남북한의 의료지식의 차이 등으로 인하여남한에서의 의료체계에 편입, 적응하기에 많은 사회경제적 어려움을 겪고 있다(박상민 외, 2011).


이들 북한이탈의사들의 남한에서 의사되기를 위한 교육은 여러면에서 중요하다.

  • 첫째, 향후 통일을 대비하여 남북한 의료통합을위하여 남쪽과 북쪽의 의학지식을 모두 알고 있는 전문가들의 양성이 필요하다는 점, 
  • 둘째, 북한이탈주민의 진료는 다른 의료사회문화적인 배경을 이해하여야 하는데 이에 대하여 최전선에서 진료하고 해석할 수 있는 역할자로서의 의료진을 발굴해 낸다는 점, 
  • 셋째,북한이탈의사들이 남한에서 의사로 정착하는 과정에서 어떤 자격인정과정이 효과적일 것이며(북한에서는 정규 의학대학을 졸업했거나 의학대학 통신학부 졸업자 중 의사검정시험에 통과한 경우 의사자격을 인정한다. 고등의학전문학교 및 의학전문학교에서 부의사와 준의사를 양성하였다. 의사들은 보건성이 3년에 한번 주관하는 시험에 따라 6급에서부터 1급까지 한 단계씩 진급할 수 있다[이윤성 외, 2011]) 어느 그룹이 어느 정도의 재교육과정을 거쳐야 하는지를 가늠할 수 있는 잣대가 된다는 점에서 중요성을 갖는다.


2012년 현재 북한이탈주민의 수는 더 증가하는 추세를 보이고있지 않지만 전문직종의 이탈주민은 늘고 있다. 이들의 재교육, 특히 의료진의 교육문제가 매우 중요하다는 인식하에 ‘남한에 정착한 북한의사의 의료전문직 재교육 프로그램 개발 및 통일 후 남북한 의사인력 통합 방안 모색,’(박상민 외, 2011) ‘북한이탈주민에 대한 보건의료인 자격 인정 방안 마련을 위한 기초 연구’(이윤성 외,2011) 등의 고찰연구들이 이루어졌다. 앞선 연구들이 국외 사례의분석과, 자료연구와 북한이탈의사들의 인터뷰를 통하여 이루어진결과물이라면, 본 소고는 서울특별시 서울의료원에서 2009년부터2012년 현재까지 북한이탈의사들을 재교육하였던 경험을 정리한것으로 이들에 대한 가능한 교육방법의 도출과정의 실제 경험, 피교육생들의 필요와 상황에 따라 확장된 일련의 과정에 대해 소고로 제시할 수밖에 없는 한계를 지닌다. 그럼에도 북한이탈의사들에 대한 현재 상황에서 가능한 교육체계의 구축과정과 그 가운데드러난 문제점, 이탈주민의사들에 대해 교육에 참여하고 함께 임상에 노출되었던 남한 의료진들의 관점에서 본 피교육생들에 대한평가, 시험의 결과를 통하여 본 이들의 현재의 지식과 의학적 의사소통 가능성에 대한 관찰결과의 제시를 통하여 향후 구조화된 교육지원체계의 구축, 의사자격 인정방안 모색, 더 나아가 남북한 의료 통합을 위한 기초자료로 활용이 가능하겠다. 특히 2013년 통일부에서 제2하나원에서의 전문인력 재교육을 계획하고 있는 시점에서 향후 관련 기관 간에 발전된 논의를 촉발하길 기대한다.




교육대상자의 선정


서울의료원은 2009년부터 2012년 현재까지 북한이탈의사의 한국의사고시 준비 교육과정을 지원하고 있다. 북한이탈자 보호 및정착 지원에 관한 법률에 근거하여 학력인정신청서를 통일부에 제출하면 교육과학기술부로부터 북한이탈주민 재북학력 인정과정을 거쳐 6년제 의과대학 졸업자와 동등한 학력을 인정한다. 학력을인정받은 북한이탈주민이 다시 통일부에 자격인정신청서를 제출하게 되고 통일부는 보건복지가족부에 응시자격인정 심사를 요청하고, 보건복지가족부는 한국보건의료인국가시험원(국시원)에 심사를 의뢰하여 인정심의위원회의 구술평가를 통해 응시 적격 판정을내리게 되고 이 대상자는 국시원이 시행하는 보건의료인 국가시험(의사고시)에 응시할 수 있다. 응시자격인정 심의결과는 2011년까지의사 33명 중 23명이 인정되었다. 응시자격인정자들의 국가고시 응시결과는 22명 지원에 8명(36.4%)이 합격하였다(이윤성, 2011). 본원에서는 응시자격인정 심의를 거친 북한이탈의사들에게 학력인정증명서 및 이력서, 자기 소개서를 제출하도록 하여 신분과 학력을재확인 후 대한민국 국가고시의 준비과정을 교육제공 및 행정지원을 통하여 돕고 있다(표 1).






사업의 내용


전체적인 교육 프로그램 지원의 구조와 각 기관의 역할은 그림 1과 표 2에, 교육계획의 예시는 표 3에 제시하였다.


본 의료원에서는 2009년부터 공공보건의료 시행계획의 일반사업 중 하나로 ‘각종 특수 계층의 다양한 교육지원’을 계획하여 특수취약 계층의 전문인력 중 본원에서 및 다양한 형태의 교육지원을 원하는 자를 교육대상으로 하였다. 주요 진료과들의 순환형 실습교육및 시험, 각종 임상술기에 대한 교육과정 지원, 시뮬레이션 지원(서울의료원 기본 심폐소생술 교육, 서울 성모 스마트센터 협력), 대학병원들과 연계하여 5회의 모의 국가고시에 참여하도록 하였다. 원내에 전용공부방을 설치하고, 의학도서관 사용, 실습용 임상가운제공, 격려 장학금지원, 유관인력 간담회(통일부 정착지원과, 질병관리본부, 하나원 의료진, 서울대, 고려대 교수진과 자문)를 실시하였다. 결과로 4명의 교육생 중 1명이 면허를 취득하였고 2명이 실기시험에 합격하였다. 면허 취득한 1인은 본원에서 인턴 수료하였다.


2010년 교육과정은 2009년의 교육내용과 구축된 연결망을 기반으로 하였다. 대상자들의 시험결과를 기준으로 학습 요구사항이달라 필기 준비팀 2명과 실기, 필기 동시 준비팀 3명의 두 팀으로 구분하여 일정을 마련하였다. 필기시험 준비 정리 및 실기시험의 체계화를 위해 통일부의 지원을 받아 연세대학교 의과대학과 연계하여의과대학 본과 4학년 필기 준비 블록강의 및 시뮬레이션센터 임상실기교육을 받도록 하였으며, 의과대학 컨소시엄과 연결하여 필기및 실기시험을 경험하게 하였다. 원내 임상교육을 강화하였으며, 본원의 인턴 및 레지던트 중 인력을 선정하여 대상자와 국가시험 종결까지 1대 1 멘토링 프로그램을 지원하였다. 하반기에는 한국 누가회의 북녘사랑 단체가 임상교육과정 중에 포함되지 않는 예방의학, 의료법 교육 및 실기 집중훈련에 도움을 주었다. 5인의 교육생중 1인이 면허를 취득 후 본원 인턴 선발되어 수료하였으며, 2인이실기에 합격하였다. 한 해의 공부로 합격한 대상자가 1명, 2년에 1명으로 통일부의 지원이 1년에 국한되어 있던 것을 지원확대 요청하여 3회까지로 위탁교육비지원을 확대하였다.


2011년에는 지방에 사는 대상자들(G씨, H씨)의 참여로 교육환경뿐 아니라 숙식할 수 있는 환경이 필요하여 지원을 요청하였으나현재까지 미결과제이다. 차선으로 원내 당직실을 공부방으로 이용할 수 있도록 하여 일부 숙식 가능하도록 하였다.


2012년에는 필기시험에 합격한 대상자 F씨로 부터 실기시험 전까지 원내 임상실습에 대한 요청이 있어 의과대학에서의 임상실습형태로 임상과 순환교육을 실시하였다. 실제 국가고시 필기시험에합격하였고 병태생리에 대한 이해가 많은 대상자임에도 임상실습에서 많은 내용을 처음 접한다고 응답하여 향후 의사고시 합격한대상자들에게 이런 형태의 임상연수교육이 임상의로서 일하기 전에 필요할 것으로 사료된다. 국가고시 응시자격인증까지의 기간이오래 소요되어 학력인정 이후 인정심의위원회 대기 중인 대상자들(I씨, J씨)이 본원의 교육생으로 지원하였다. 이에 재정지원이 필요한 원외교육을 제외한 원내 교육 및 멘토링 프로그램에 참여시켜교육 중으로 차후 대상자의 선정 및 지원범위 문제에 대한 논의가필요하다.








세부사항


1. 행정적 지원


지속적인 프로그램의 운영을 위해 가장 중요하였던 지원으로 1,2차년도에는 공공의료팀 소속의 사회복지사가, 2차년도 이후부터현재까지 1인의 공공의료팀의 사무관리사가 담당하였다. 교육생의선정 및 서류관리, 연간사업계획서 및 연간결과보고서 작성 및 관리, 교육생 일정관리, 사업지원비 청구(시보조금 및 통일부), 연세대 의과대학 교육비 지원업무, 타의과대학 및 컨소시엄과 연계를통한 모의고사지원, 행정적 지원(가운, 식권, 공부방 마련, 도서실이용 등), 만족도조사, 멘토링 프로그램 체크, 기본심폐소생술 교육지원, 국가고시 교재지원, 교육생 조건부 수급자 서류제공 등 서류작성업무 등을 지원하고 있다.


2. 원내 교육


원내 임상교육의 목표가 조정되었다. 첫 해인 2009년에는 실기및 필기시험 대비뿐 아니라 남한에서의 의료시스템의 전반적인 경험을 하는 것을 목표로 실습하였다. 그러나 단기간의 준비기간 내에, 경제적인 어려움 속에 아르바이트를 하면서 공부하여야 하는상황으로 빠른 성과가 필요하여 목표를 국가고시 통과로 축소하였고, 임상실습을 통한 남한 의료제도 적응의 목표는 인턴 이후 과정으로 넘겼다. 공부를 진행하는 중 수급비지원이 끊기게 되므로 이시기의 생계유지를 위해 병원에서 짜준 고정된 스케줄에 교육을받는 것이 실제로 어려웠다. 교육 스케줄의 유연한 변경 운영이 필요하였으며, 교육생의 개인적인 요구와 성향에 따라 참여도도 차이가 있어 일괄적인 평가는 어려웠다. 아무런 특별한 지원이 없는 상황에서 자발적으로 참여한 원내 임상과별 이론교육(문제풀이, 이론강의)은 계획의 수시 변경이 많았으나 교육현장에서 직접 질의가 가능해서 내용을 이해하는데 도움이 되었다고 하였다. 원내 교육 중 영상의학과의 경우 지속적으로 진행되었다. 북한에서 임상의로서 경험하지 못하였던 엑스레이, 초음파, 전산화 단층촬영의 판독법, 임상에서 중요한 영상자료 등에 대하여 주 1-3회 정도 1시간씩 교육을 지속적으로 실시하여 오고 있어 교육자의 노하우도 쌓이고 교육생의 만족도도 가장 높았다. 통일부와 서울시의 보조금예산은 전액 외부교육 및 시험 관련 비용으로 배정되어 맞춤형 원내 프로그램의 개발이나 유지를 위한 고려가 없이 원내 자원봉사형식으로 운영되어 지속 운영과 관리에 어려움이 많았다.


3. 원외 의과대학 위탁교육


본원 특성상 연구교육보다 임상에 집중하고 있으므로 기존 운영되고 있는 대학에서의 교육 프로그램에 대한 벤치마킹 및 북한이탈의사에 맞는 프로그램의 개발이 필요하지만 재원 등이 확보되어 있지 않아 의과대학과의 상시적 연계협력이 필요하였다. 2009년에 개별적 단편적으로 대학들과 연결하였던 것을 2010년부터 연세대학교 의과대학과 업무협약을 맺어 교육연구부, 임상실습센터와 연결하여 이론과 실습교육을 받고 있다. 의과대학에서의 본과 4학년 필기대비 블록 특강교육은 전체적으로 교육내용은 많이 도움이 되었으나 영어, 전문용어, 통일되지 않은 약어로의 강의가 많아 교육생들이 이해하는데 어려웠다고 한다. 시험 준비기간이 1년 이상 되는교육생의 경우 교육 참여로 전반적인 내용의 정리가 되었다고 하였으나 학업 초기의 교육생들은 시간낭비라고 판단하여 참여 중 임의로 중단하기도 하였다. 서울의료원의 진료환경이 실제 임상을 익히기에는 더 용이하나 시험 자체는 별개이므로 실기시험에 이용되는모의기구나 인형, 모듈이 시뮬레이션 센터에서의 교육, 준비, 모의시험과정에 제공되어 시험과 유사한 환경을 익히는 것이 필수적이었다. 또한 국가고시가 결과에 대한 피드백이 없는 것과 대조적으로부족한 부분에 대한 피드백이 가능하여 실제적인 도움이 되었다.


4. 필기 및 실기 모의고사 참여


1차년도에는 개별적으로 의과대학에 연결하여 시험에 참여시켰으며 무료로 위탁하였던 것이 2차년도부터는 의과대학 컨소시엄과 연결하여 관계된 여러 학교에서 실시되는 필기 및 실기 모의고사에 참여할 수 있도록 하였다. 실기의 경우 본원 담당자가 같이 동행하였고 컨소시엄 담당 교수님들은 담당자가 직접 상황을 보고문제점을 볼 수 있도록 협조해주셨고, 피드백을 주셨다. 시험환경에 익숙해지는 것이 중요하므로 교육생들은 시험의 기회가 여러 번주어지기를 원하였다.


5. 멘토링 프로그램 운영


개인별 학습능력에 따른 지원이 쉽지 않고 본인 스스로 학습에대한 어려운 점을 이야기 하지 않으면 도움을 주는데 많은 제약이있고, 사업 담당자가 수시로 교육생들의 학습 진행사항을 관리하기에는 한계가 있어 수련병원인 서울의료원의 인적자원(인턴과 전공의)과 연결하는 멘토/멘티 연결 프로그램 ‘Each &Together’를진행하였다. 멘토와 멘티는 유무선상으로 수시로 연락하여 학습과정 중의 문제의 해결과 시험 준비의 기술, 실습 등의 도움을 주고받았다. 멘토와 멘티의 성향에 따라 학습효과와 만족도의 차이가 매우 컸다. 인턴의 경우 최근 실기시험 준비과정의 경험이 있고 임상술기를 보여줄 수 있었고, 전공의의 경우 필기시험 준비의 내용에대한 이해를 도와줄 수 있었다. 기본적으로 2인 정도의 멘토가 연결되는 것이 연속성을 담보하였다. 이 외에 기존 경험 있는 멘토를다음 해 슈퍼바이저로 하였고, 본 교육과정을 이수하고 인턴과정에 있는 북한이탈주민 의료진을 볼런티어로 하여 돕도록 하였다.


결과 및 이후 과정


1. 시험결과를 통한 평가


2010년 일개 의과대학 임상종합추론 블록에 임한 5명의 교육생(국시 준비기간 3개월-1년 6개월)의 성적은 48.23±10.92의 의과대학 학생 평균 대비하여 34.38-63.54점까지의 분포를 보였다. 2011년임상종합평가결과의 경우 한 교육생의 경우이지만 백분위 점수는59점이었으나 정신과와 예방의학, 의료법에 특히 취약하였다. 예방의학과 의료법의 경우 별도의 교육이 필요하다. 필기 국가고시의 성격상 병태생리의 이해, 진단과 치료의 전반적 내용을 다루므로 준비 첫 해에는 용어의 어려움으로 힘들어 하나 두 번째 지원에는 대부분 필기합격점에 이르렀다. 임상술기, 환자면담시험의 경우 북한에서의 임상과에 따라 술기능력의 차이를 보였으나 해당 임상과가아닌 경우 검사를 충분히 숙지하지 못하였다. 동맥혈검사, 심전도검사, 기관 삽관법, 드레싱 등의 술기에 대해 더 많은 실습이 필요하였고, 금연상담이나 예방접종상담, 환자교육의 부분이 취약하였다.


2012년 시험에서는 필기는 합격하였음에도 실기가 탈락하는 예가 발생하였다. 실기시험 시 총점 자체는 충분하였으나 항목별 점수를 받지 못해 탈락하였다. 변형된 문제유형에 대한 대비가 더 필요하고, 북한 말씨가 판단에 영향을 주지 않으므로 자신감이 필요하다는 정도의 전반적인 국시원의 평가 의견을 받았다.



2. 교육에 참여했던 남한 의료진들로부터의 피드백 결과


임상과 교육에 참여하였던 전문의와 전공의, 멘토로 참여한 인턴선생님들에게 설문형식으로 피교육생들의 교육내용에 대한 이해도를 개방형 질문을 통하여 평가하였다. 설문참여자는 남녀 12명(전문의 7명과 인턴, 전공의 5명), 평균 연령이 37세였다.


결핵, 내과계 일반(호흡기, 순환기, 소화기계)질환, 외과계 일반질환, 분만 등의 내용에 대한 기본적인 병태생리적 이해를 갖고 있는것으로 판단되나, 이에 대하여 숙지하고 처방이 이루어졌던 경험은드문 것으로 보인다. 내과에 근무하였다 하더라도 진단검사, 영상의학검사나 내시경적 진단을 기능 진단과에서 시행하고 임상의사는이를 읽고 해석하거나 시술하지 않아 검사 자체나 결과를 처음 접한다고 하였다. 영상의학, 정신과, 기기나 각종 시술의 이해(예, 심전도),혈액종양내과 등에 대하여는 내용을 처음 접한 것으로 인식된다.


임상술기에 해당하는 비위관 삽입, 복수천자 등의 경험이 없었고 상처 치료에 있어서 무균술 개념의 이해가 부족하였다. 그러나산부인과 분만, 신경과 검진 등의 경우 실제 해당 임상과에 오래 근무한 북한이탈의사들은 안정적인 술기를 보여주었다.


개인적인 역량, 이해도 및 임상능력의 차이가 명확하였다. 북한에서의 의사급수나 근무경력, 시기 등이 영향을 주는 것으로 보인다. 기본적인 의학지식의 정도와 이해도에 대하여 정확한 파악이어려워 어떤 내용을 어디까지 교육해야 할지 결정하는 것이 어려웠다고 하였다. 교육생들은 기본적으로 학습내용의 기둥을 세우고여기에 덧붙이는 것을 중요한 학습법으로 이해하고 있었다. 따라서교육생 개인이 먼저 공부하고 문제 풀이한 후에 설명해 주는 방식보다 기본적인 골격을 먼저 설명하여 주고 공부하도록 하는 것이바람직하겠다.


기본적인 용어로 영어 사용은 매우 커다란 장벽이었다. 40대 이상의 의료진들에 비하여 30대의 의료진들, 러시아어 대신 영어를외국어로 선택하였던 교육생들은 기초적인 영어습득의 경험이 있고 비교적 쉽게 영어표현에 적응하였다. 한글의학용어가 통용되고있다고 하나 표현의 차이들이 존재하여 용어의 재정립이 필요하였다. 의학영어교육과정과 북한의학용어와 남한의학용어, 라틴어, 영어 등의 비교사전이 마련되어야 한다.


개인차가 크므로 멘토링 프로그램이 효과적이나 동기부여나 보상체계가 없어 이에 대한 고려가 필요하겠고 책임 있는 멘토링과 관리, 결과의 피드백이 필요하였다. 복수 멘토(시험을 바로 치룬 인턴과 임상과를 설명할 수 있는 레지던트 또는 전문의)가 효과적인 도움이 된다. 지방에서 거주하시는 분이 공부에 전념할 수 있는 숙소마련이 시급하며 국가고시 준비 프로그램 이후의 사후 프로그램이필요하다. 또한 현재처럼 국가고시 응시자격인증이 명확한 기준이없고, 그 절차가 오래 걸리는 상황이 지속된다면 교육생의 선발기준이 재정립될 필요가 있겠다. 맞춤형 원내 커리큘럼의 개발 및 유지를 위한 지원이 필요하며 이 프로그램은 현장을 충분히 반영하여 이탈주민 의료진과 교육 경험자들이 함께 준비하여야 한다고대답하였다.



3. 국가고시 통과 이후, 인턴 및 레지던트 지원과정


총 4명이 합격하였고 이 중 A씨와 B씨 2명이 2010년과 2011년에서울의료원 인턴에 선발되어 수련 받았다. 수련의의 지원문제에서의과대학 졸업증명서와 성적증명서의 서류 미비로 이를 가름하는다른 행정적 절차가 필요하여 통일부의 협조공문을 요청하였고 내부방침을 받았으나 향후 절차가 요구된다. 졸업증명서를 통일부의학력인정서로 대신하고 성적증명서가 없으므로 응시생 평균점수등으로 갈음하는 등의 방침이 필요하다. 인턴수련 초기에는 어려움을 호소하였으나 술기 습득능력이 우수하고 태도가 성실하여 모두B등급의 평점으로 수련과정을 마쳤다. A씨의 경우 레지던트의 지원과정에서 서류 미비의 문제가 있었다. 외과 지원과정에서의 실패로 인한 거절감, 응급의학과 일반의 근무 중의 근로여건 등에 대한문제로 그만두고 현재 일반의로 일하고 있다. B씨의 경우 우수한 시험성적과 인턴성적에도 본원 가정의학과 지원에 실패하였으나 H병원 가정의학과에 재지원하여 자신을 피력하고 과의 이해를 통해선발되어 현재 1년차로 생활하고 있다. 힘들긴 하지만 수련의의 길에 들어서길 잘하였다고 진술하였고 역시 영어에 어려움을 호소하고 있으나 주변 의료진에게 좋은 평가를 받고 있다.


2012년 지원에서 57세의 H씨가 서울의료원 인턴선발에 지원하였으나 불합격하여 2차 지원과정을 본원 공공의료팀에서 지원하였었다. 그러나 의사소통과 신뢰 부재로 직접 타지역병원에 지원하여인턴과정에 있다. C씨는 H씨와의 본원 동시에 지원하는 것을 포기하고 인턴을 지원하지 않고 현재 봉직의로 일하고 있다. 시험 준비기간이 짧고 남한의 의학 관련 용어에 익숙하지 못하고, 상대적으로 다른 수련의 지원자들보다 나이가 많은 북한이탈주민 의사들이우수한 의료기관에 수련의로 지원하여 교육받기에는 의료기관으로서의 부담이 적지 않고 타지원자와의 형평성 문제가 발생한다.슈퍼비전이 가능한 자원하는 국공립의료기관이나 관심 있는 대학병원에서 군 위탁교육 이후 의무부대에 근무하는 것과 같은 형식으로 수련의의 추가인력으로 수련 자리를 꼭 확보하고 북한이탈주민 의료협력센터가 구축되어 있는 기관에서 북한이탈주민을 진료하는데 직간접적으로 근무 활동하거나 통일학 분야에서 의학의 통일 전 기반구축연구에 참여하도록 하는 조건을 두는 방안들이 필요하겠다.


결론 및 제언


통일 이후 어떤 형태에서든 남북 의료제도의 통합이라는 문제가해결되어야 한다. 보건의료제도뿐 아니라 의료인력의 교육 및 면허부여과정의 통합이 중요한 과제이다. 

  • 이스라엘의 경우 1990년대 초반 사회주의권 붕괴와 함께 구소련 출신 이민자 의사들이 이스라엘로 유입되었을 때 이스라엘 정부는 친이민정책인 귀환법에 따라의사직종에도 정부의 지원이 이루어졌다. 준비기간 동안의 생계를위한 생활비를 지원받으면서 1년에 걸쳐 히브리어, 의료용어에 대한 교육, 최신의료추세와 임상수기에 대한 기초지식을 향상시킬 수있도록 의사면허 준비과정을 제공받았다. 또한 이스라엘 보건부는정부재정을 투자하여 전국의 병원들에 이민자 의사들을 위한 상당수의 레지던트 자리를 추가 배정하였다. 
  • 통일 독일의 경우 통일이전 동서독 의학교류가 어느 정도 활발하게 있었고 동독에도 분단 전의 의학제도가 존속되어 있었으므로 통일 후 의사면허를 그대로 인정하였다. 그러나 의료장비 및 기술의 차이뿐 아니라 여러사회경제적 문화적 장벽이 지속되어 통합의 어려움을 겪었고 동독출신 의과대학 학생들의 경우 1990년 이후에는 서독의 규정에 따라 의사실습과정을 거친 이후에만 의사면허가 부여되었다(박상민외, 2011).


대량 탈북 이전의 통일 준비시기에서의 북한이탈의사들에 대한일개 기관의 교육의 경험을 통해 현 교육과정의 문제점과 향후 개선방안을 다음과 같이 제언할 수 있겠다.


첫째, 의사면허의 준비과정을 위한 교육 프로그램과 임상활동을위한 실습 프로그램이 개발되어 시범적으로 지역적으로 나뉘어 운영될 수 있도록 체계적이고도 제도적인 지원이 필요하겠다. 임상현장에서 피교육생의 요구에 따라 현재까지의 과정이 제공되었으나전반적인 의학교육의 목표설정과 장기적인 남북 의료통합이라는전반적인 맥락에서 의학교육 전문가와 병원 의료진, 통일문제 전문가가 같이 준비하여야 한다. 북한이탈의사들을 남한 의사시험에합격시킬 것인가 아니면 남한의 의료시스템에 적응시킬 것인가? 이들의 역할을 무엇으로 할 것인가? 일반 시장경제 의료제도에 편입될 것인가 아니면 북한이탈주민의 진료, 남북 의료통합의 과정에서어떤 역할을 부여할 것인가? 이들의 역할을 제한한다기보다는 장점을 활용하여 단기적으로 다른 의료사회문화적 환경을 가진 북한이탈주민을 진료하는데 직간접적으로 활동하거나 통일학 분야에서 의학의 통일 전 기반구축 연구에 참여하게 한다면 효과적일것이다. 최근 북한이탈주민이 지방에 배치되고 정착되는 경우가 있어 한 기관에서만 교육을 담당할 것이 아니라 지역적으로 몇 기관으로 분산되어 교육과 실습이 진행되는 것이 바람직할 것이다.


둘째, 다른 학습목표와 커리큘럼으로 공부하고 임상에 임해왔던 북한의 의료진들을 평가한다는 것은 쉽지 않다. 조심스럽지만의학지식에 대한 실제적인 평가연구가 선행된다면 추후 이들의 자격을 인정하는 기준을 현재대로 인정 심의과정을 거쳐 의사 국가고시를 보도록 할지, 1년 정도의 단기 재교육으로 인정할지, 의과대학에 편입 후 국가고시를 보도록 할 것인지 등의 여부를 결정하는데 도움이 되겠다. 방법으로는 의과대학 학습목표에 대한 지식의정도 확인과 그에 대한 임상경험, 처방의 경험 등을 확인하거나 구조화된 동일한 설문을 남한 의과대학 졸업반, 수련의들과 함께 평가 비교하는 방법 등이 있겠다. 차이가 있는지, 있다면 그것이 의학지식의 차이에 의한 것인지, 아니면 사회경제적 악화로 인한 기자재의 부족, 처방 불가 상황이나 왜곡된 의료시스템의 문제에 의한 것인지 의료진의 개개인 차이에 의한 것인지 평가하는 작업이 필요하겠다. 현재의 국가고시 응시자격인정을 위한 인정 심의제가 선택한구술평가방법이 구체적인 준거와 기준을 가진 평가라 보기 어려워더 구체적인 기준과 항목이 필요하다는 지적이 있었다(이윤성 외,2011). 최근 인정 심의신청자가 늘었지만 합격률이 급격히 떨어지면서 북한이탈의사들 내부에서도 인정 심의결과에 대하여 의문을제기하고 있다.


셋째, 남한의 의학교재를 이용하여 지식을 습득할 수 있기 위해서는 의학용어의 교육이 필요하다. 적어도 이들의 교육을 위해 시험 준비를 위한 필수 의학용어에 대한 남한 의학용어, 영어, 북한의학용어(가능하다면 라틴어, 러시아어 포함) 대조 의학용어집의편찬이 먼저 필요하겠다. 남한의료진뿐만 아니라 먼저 의사가 된북한이탈의사들이 함께 참여한 작업이 필요하겠다. 이후 이는 남북한 공통의 의학용어사전의 편찬작업에 도움이 되리라 판단된다.


넷째, 인턴 및 전공의의 수련의 정원 확보가 현재로서 시급하다.북한이탈의사 교육은 의사로서 살아가도록 돕는 전반적인 과정에서 다루어져야 한다. 현재 제도에서 이들이 국가고시에 합격하더라도 개별 경쟁해야 하는 구조로 다른 남한 의과대학 졸업생 경쟁자와의 형평성의 문제, 실제 능력의 차이 극복을 위한 시간이 없다는점, 병원이 추가적인 이득 없이 부담을 수련의의 선발과정에서 그대로 감수하여야 하는 점 등의 문제가 발생한다. 1차 지원에 탈락하게 되는 경우 이들은 2차, 3차 지원을 통하여 수급이 비는 지방으로내려가 인턴을 수료한 후 봉직의나 개원의로 근무하거나 바로 봉직의로 일하는 형식을 택하므로 사후 교육에 문제가 발생한다. 이스라엘 정부가 구소련에서 귀환 의사들의 정착, 통합을 위해 전공의수련 자리를 마련하였었던 것을 선례로 국내에서도 우선은 경험과관심이 있는 공공병원, 대학병원들에 추가적인 수련 자리를 보건복지부, 통일부와 논의하여 마련하고 수련을 보장하는 것이 제도적으로 선행되어야 하겠다.





As North Korea passed from the Devotion (Jeongseong) movement to the black market (Jangmadang) system,

the medical service system in that country was effectively destroyed. North Korean physicians who have successfully

defected to South Korea (North Korean defector physicians, NKDPs) have experienced socio-economic

hardships on their way to becoming incorporated into the South Korean medical system due to different medico-

social cultures, different (English-based) medical terminology, and the clinical knowledge gap between

North and South Korea. Since 2009, we have operated programs at the Seoul Medical Center to help NKDPs

prepare for the South Korean medical licensing examination. These programs consist of clinical education at the

medical center, personal mentoring, arrangement of educational programs at the medical college, mock tests

at the consortium, and administrative aid. Looking forward, we hope to achieve the following: 1) More systematic

support plans are needed involving medical education experts, field physicians, and experts on reunification.

2) An evaluation of defector physicians’ current medical knowledge may provide information about the areas

where supplementary education is most needed and the standards for certificating licenses. 3) In the short

term, a customized glossary should be developed to assist defector physicians prepare for the examination. 4)

To secure internships and residencies is the most important issue for further sustained training of NKDP physicians

to become good clinicians after certification. Hopefully, this short report on the current ongoing educational

course will lead to more extensive discussion.

Partnering with academic institutions

 

GHS’s collaborations with academic institutions in the United States, Asia, Europe, Latin America, and Africa provide us with a platform from which to develop new knowledge useful to the global health community and to provide opportunities for faculty and student exchanges between UCSF and these institutions.


 The Global Health Faculty Scholars Program described above is one element of a strategy to build long-term institutional relationships with international academic institutions, most of which are located in low- and middle-income countries. 

Some of these institutions, particularly those in sub-Saharan Africa, are struggling to increase their output of qualified graduates to address the health workforce crisis in their countries. Inadequate academic infrastructure and an insufficient number of faculty members make this a daunting task. 


UCSF와 같이 대규모의, 많은 자원을 가지고 있는 기관이, 자원이 부족한 보건관련 기관과 협력하는 것은 장기적으로 두 기관 모두에게 목표 달성을 위한 좋은 전략이 될 수 있다. 이는 흔하게 있는 학문적 관계, 즉 개개 교수, 프로젝트, 학과 차원의 것과는 성격이 다르다.

We believe that for an institution as large and well-resourced as UCSF, forming partnerships at the institutional level with less-well-resourced schools of health sciences can be a step toward developing long-term strategies that meet the goals of both institutions. This approach contrasts with typical international academic relationships, which are more often at the individual faculty, project, or department level.


 

Since 2005, we have been working with Muhimbili University of Health and Allied Sciences (MUHAS) in Tanzania to develop a comprehensive institutional partnership model.9 The components of the model include building cross-campus health sciences cores at each institution to introduce educational innovations across all professional degree programs; arranging sandwich programs that match the needs of students in different MUHAS departments and schools with the skills and expertise available across the UCSF campus; providing institution-wide faculty and student exchange programs between our institutions; developing exchange programs for senior administrators; and creating multidisciplinary collaborative research programs.


Addressing neglected global health issues

The promotion of surgery as a much-overlooked but cost-effective strategy to prevent morbidity and mortality in many low- and middle- income countries is a major priority area that is unique to GHS. Our research has highlighted that, especially in the care of injury victims and obstetrical complications, certain surgical interventions provided in district hospitals can significantly reduce disability-adjusted life years at a cost per person comparable with that of providing immunization.10,11 A strategy is needed to create awareness of national governments and international agencies to raise essential surgery on their agendas, to generate new research, and to translate our knowledge into universal interventions that impact morbidity and mortality. 


In June 2007, in collaboration with the Karolinska Institute, the Fogarty International Center, World Bank, and the World Health Organization, we cohosted a meeting on increasing access to surgical services in resource-constrained settings in sub-Saharan Africa at the Rockefeller Foundation’s Bellagio Conference Center. The meeting formed The Bellagio Essential Surgery Group with representation from Kenya, Eritrea, Mozambique, Southern Sudan, Tanzania, and Uganda. Building on experiences such as those gained from training assistant medical officers, or Técnicos de Cirurgia, in Mozambique,12 this consortium agreed to develop health services research and surgical training programs in underserved regions of sub-Saharan Africa and to build infrastructures that will make surgical services more accessible at the district hospital level. Through our GHS Essential Surgery program, we will continue to support these efforts and to conduct further research.

The world has become increasingly interconnected and globalization now affects virtually every person’s life. Increases in the flow of people, products, services, and information between and among countries and continents are having a dramatic influence on the world’s health and health care delivery.1 The global migration of people and the distribution of products increases the threat of communicable diseases, such as HIV/AIDS and SARS, as well as the rapid spread of avian influenza and multidrug-resistant microorganisms.2,3 In today’s society, the emergence of a new public health threat in one part of the world becomes a concern throughout the world.2

Research conducted in resource-poor settings has helped answer many questions relevant to medical care in developed countries, but current research funding disproportionately favors studying the diseases of high-income countries over studying those diseases more prevalent in low- and middle-income countries.4 Whereas injury and environmental health problems are persistent concerns in resource-poor settings and chronic noncommunicable diseases are continuing to rise, further efforts are needed to understand the contextual basis for poor health among these communities.5,6 To address the United Nations’ Millennium Development Goals, research efforts are being specifically directed to better understand how to strengthen research capacity in low-income countries, sustain the emerging public–private partnerships, and integrate equity and gender issues.7,8


Increases in the capacity and exchange of health services and information can be used to better address global health threats and influence research priorities.5,9 Medicine and public health must continue to become more globalized so that by addressing the emergence and distribution of diseases in low- and middle-income countries, the health of communities in high-income countries is promoted as well.

In addition, addressing global health needs, especially among poorer countries, will not only help promote economic development but may also reduce health inequalities and foster political stability and security.10–13 These were some of the reasons the Institute of Medicine recently called on the federal government to create an organization to mobilize U.S. health care workers in the fight against HIV in developing countries.14

Global Health Demands on New Physicians

In the era of globalization, physicians are now expected to have a broader understanding of various alternative and culturally determined medical practices, as well as knowledge of tropical diseases and emerging global infections.15

The steady increase of travel and migration has increased clinical visits for travel medicine, including immigrants visiting their home country.16,17 

    • Hence, a clinician’s ability to recognize or suspect presentations of diseases endemic to other world regions has become increasingly important.18 
    • Physicians must also learn about determinants of health and disease, including socioeconomic, environmental, and political factors, which are becoming more globally interconnected.15 
    • New physicians will also be facing more cross-cultural interactions and must be comfortable working with translators and understanding cultural beliefs among different ethnic groups.18 


치유(Healing)에 대한 서로 다른 문화적 접근의 차이는 ethnomedicine 분야에 잘 정리되어 있다.

The complexity of the interplay between different cultural approaches to healing has been well documented in the field of ethnomedicine 19 as well as in the popular book The Spirit Catches You and You Fall Down.20 


또한 선진국에서 의사들은 의료적으로 취약한 사람, 보험이 없는 사람들의 요구에도 맞춰줄 수 있어야 한다.

Furthermore, in developed countries, primary care physicians must meet the needs of the medically underserved and uninsured, who suffer increased disease morbidity 21 and whose needs could be better addressed if health care professionals had a better understanding of global health. 


요약하자면, 새롭게 양성되는 의사는 국제보건이슈에 익숙해져 있어야 하고, 기존의/새롭게 등장할 질병을 이해해야 하고, 다양한 문화에 대한 이해와 감수성을 가지고 있어야 한다.

In summary, newly trained physicians need to be well rounded on global health issues, understand existing and newly emerging global diseases, and be cross-culturally competent and sensitive.

Education for Global Health in Medical Schools

 

미국의 의과대학생들은 50년 이상 개발도상국에서의 임상실습에 참여해왔고, 최근에 그와 관련된 흥미와 참여가 더 많아지고 있다.

Medical students in the United States have engaged in international rotations in developing countries for over half a century,22 and their interest and participation has accelerated in recent years.23 

In 1978, 5.9% of graduating American medical students had completed a clinical education experience abroad as part of their medical education (Figure 1).23
By 2004, 22.3% of graduating American medical students had participated in an international health experience.24 


그러나 효과를 최대한으로 끌어올리기 위해서는 이러한 해외 임상실습이 이에 대한 교육과정과 포괄적으로 통합되어야 한다. 의과대학생들은 international clinical rotation and research opportunities를 위해서 많은 시간을 투자하고 있으며, 의과대학생은 국제보건 문제를 좀 더 강조할 것에 대한 요구를 이끌어나가고 있다.

However, for maximal effect, international clinical rotations need to be integrated with a comprehensive international health curriculum.25 In 1991, only 22% of U.S. medical schools offered a course on international health.26 In addition, many medical students are now expanding the time they spend in medical school to pursue international clinical rotations and research opportunities. Medical students have been leading much of the call for greater emphasis on global health issues as part of medical education.






현재 거의 모든 대학이 의과대학생들이 국제보건에 대해 흥미를 갖고 활동할 수 있는 것을 마련해주고 있다.

Currently, almost all medical schools have some avenues for medical students to pursue global health interests or activities (P. Gardner, personal observation, 2006). 

    • At a minimal level, the great majority of medical schools have a student-led interest group to discuss various global health topics, often with faculty or visiting lecturers. 
    • Some schools are now requiring first-year students to choose an area of special interest and are offering global health as an option, which is proving to be highly popular. 
    • At medical schools with more mature global health programs, travel support is generally available to help medical students participate in global health projects. 
    • Furthermore, many of these schools and their affiliated hospitals have formed partnerships with foreign institutions, and some schools have funding for bidirectional exchange programs. 
    • Finally, a number of medical schools have created specific departments of global health, often in partnership with a school of public health, and now have more fully developed global health programs. 


In the past year, medical schools at Vanderbilt University, Harvard University, and Duke University, to name a few, have launched or expanded major initiatives in global health. The University of Washington was recently awarded $30 million by the Bill and Melinda Gates Foundation to support the creation of a department of global health.27 Thus, global health is increasingly being recognized as important by medical schools, and the growing interest among medical students continues to push global health into the mainstream of medical education.


 

International clinical rotation의 장점

Medical students recognize the benefits of including global health topics in the medical curriculum, as well as international clinical rotations in the training of medical students.15,28 

    • Those who have completed a rotation in a developing country have reported increased skills and confidence, enhanced sensitivity to cost issues, less reliance on technology, and greater appreciation for cross-cultural communication.22,28 
    • They become better clinicians by broadening their clinical exposure and experience, most obviously with regard to diseases that are endemic in developing countries and rarely encountered in the student’s home country. 
    • They also learn to practice medicine with limited access to laboratory tests and expensive diagnostic procedures, relying on strengthened physical examination skills and depending less on laboratory values, radiologic imaging, and other diagnostic testing, and they develop a deeper appreciation for global public health issues and become more culturally sensitive.22,28,29 
    • For instance, in-depth interviews with 24 Dutch medical students who completed an international clinical rotation revealed meaningful learning experiences in the domains of medical knowledge, clinical skills, international health care organization, international medical education, society and culture, and personal growth.30 
    • International rotations provide not only training but also opportunities for service, which can be both personally rewarding and useful for building partnerships.

International clinical rotations not only benefit the medical student, but also help to serve the needs of the health care system. 

    • Medical students and residents with international clinical experience are more likely to enter general primary care medicine.22,31–34 
    • Further, medical students and residents with international experience are more likely to obtain a public health degree and engage in community service.31,32 
    • Similarly, they embrace attitudes and desires to practice medicine among underserved and multicultural populations.31–35 
    • A two-year follow-up survey found that 23% of medical students who participated in an international clinical elective intended to work in resource-poor settings, compared with only 6% of medical students with similar plans who did not participate in an international clinical elective.36 
    • A follow-up survey of American fourth-year medical students who had completed a clinical rotation abroad found that a six-week intensive experience in a developing country influenced the medical careers of 67% of the participants; 74% were engaged in primary care specialties, and 60% planned on working overseas in the future.32

Current Opportunities for Medical Students

Although several residency programs have long offered international electives,31,34,37 earlier experiences could have an even greater impact on shaping career decisions in medicine. A few medical schools have created programs specifically to train medical students for careers in global health. 

    • In 1998, Ben-Gurion University and Columbia University founded a medical curriculum in Israel with the purpose of training physicians in global health and medicine.38 
    • More recently, the Royal Free and University College Medical School in the United Kingdom created an intercalated bachelor of science degree (equivalent to an expanded fifth year of a U.S. medical school) in international health.39

Many medical schools do offer a course or seminar on global health, and several now provide opportunities or help arrange international rotations. 

    • The Karolinska Institute in Sweden offers students an optional five-week full-time course on global health.40 
    • The University of Arizona has an international health option that allows medical students to conduct international fieldwork during their last year of clinical training.41 
    • The University of Washington recently introduced a global health pathway, which includes course work on global health and tropical medicine as well as fully funded international clinical rotations during the last year of medical school.42 
    • The University of Massachusetts offers a Global Multiculturalism Track to improve cultural competency for medical students working with local immigrants.43 
    • Several other medical schools that offer global health teaching or international clinical opportunities can be found at the Global Health Education Consortium 44 and the American Medical Student Association 45 Web sites.

Several associations, organizations, and institutions have recently created scholarships and fellowships for medical students to pursue international research and training. 

    • The Global Health Education Consortium created the Carole M. Davis Scholarship to assist medical students to complete fieldwork abroad.46 
    • The American Society of Tropical Medicine and Hygiene established the Benjamin H. Kean Traveling Fellowship to fund medical students, residents, and fellows for an international tropical medicine elective.47 
    • The National Institutes of Health’s (NIH) Fogarty International Center (FIC) and the Ellison Medical Foundation have established the Fogarty/Ellison Overseas Fellowships in Global Health and Clinical Research Training, which allow medical students to spend 10 months with an established NIH-supported research center in a low- or middle-income country.48 
      • In 2005–2006, this fellowship allowed 27 U.S. fellowship recipients and 27 matching fellows from the foreign sites to work in 18 research centers around the world. To our knowledge, this is the only one-year fellowship that supports American medical students for a year of clinical research training in a developing country. 
      • In addition, the FIC’s newly established Framework Programs for Global Health provides support for NIH-funded U.S. and foreign institutions to help develop multidisciplinary curricula in global health and encourages faculty and students from diverse disciplines, including business, law, journalism, and engineering, to work collaboratively with traditional partners in global health research.

Medical students and residents are calling for more global health teaching and international rotation opportunities during their medical school education.34,49 Nearly all medical students who have had international rotations report that these rotations are enriching experiences, and many consider them the best part of their medical education.50,51 

    • In a survey of University of Arizona medical students who completed an international clinical rotation, all 133 participants indicated that they would recommend the program to their peers.41 
    • In a more recent survey, 58 of 60 American medical students who completed six to eight weeks of field experience in a developing country said they would recommend an international clinical rotation to their peers.28 
    • A survey of Yale University internal medicine residents found that 60% of those who had completed an international clinical rotation, as well as 45% of those who had not completed an international rotation, felt that medical school training should include exposure to health care in developing countries.34

Suggestions for Developing Global Health Education


미국의 의과대학 시스템은 점점 의대생과 젊은 의사들이 국제보건 방향의 진로를 추구할 것을 권장하고 지원하고 있다. 더 많은 연구를 통해서 international clinical rotation의 효과를 좀 더 명확히 할 수도 있음에도 불구하고, 그 이점에 대한 인식은 그다지 나아지고 있지 않다. 몇 가지 제안을 하고자 한다.

The U.S. medical system has been called upon to encourage and assist more medical students and young physicians to enter global health careers.14,52 Further structured research could better elucidate the range of effects of international clinical rotations among medical students, including costs, which may include variability in medical supervision, personal safety and liability concerns, and time away from family. However, recognition of the benefits has been rather consistent. Although opportunities could be provided for students to work more with local multicultural populations, these experiences have only some, and not all, of the benefits of working in international, resource-poor settings. We now suggest several steps that can be taken by medical schools to meet the growing interest and demands of medical students for more training and opportunities in global health (List 1).

As a first step, medical schools could integrate global health topics into core medical curricula. 

    • In addition to teaching about tropical diseases and providing cross-cultural training, medical schools could also offer courses on international public health, medical anthropology, and global health economics. 
    • This could be facilitated by fostering relationships with other schools, particularly schools of public health, because medicine and public health are largely intertwined in developing countries. 
    • Additionally, schools could establish a global health pathway or track to encourage and recognize students gaining global health training and international clinical experiences. 
    • Finally, students pursuing combined degrees, such as MD/PhD and MD/MPH programs, could be encouraged to concentrate on global health.

      • The typical, and perhaps the optimum, amount of time for a clinical rotation is six to eight weeks, depending in part on pretravel logistic support. Finally, we recommend that all medical students receive training in global health and that an international clinical rotation become more routinely available to medical students. If international clinical electives opportunities remain limited and sparse, then programs will be likely to continue to draw on a small, self-selected group of students who are internationally oriented and well traveled,54 and opportunities will likely not reach those students, including members of U.S. minority groups, who would most greatly benefit from an international rotation.30 The benefits to medical students are more than sufficient to justify promoting an international clinical rotation as a worthy training opportunity.


 


Conclusion

 

Thus far, medical schools have been slow in responding to the global health interests of their students. Medical schools should be encouraged to continue integrating global health teaching into medical curricula while creating and promoting more opportunities for international rotations. Also, they should move toward making an international clinical rotation a routine part of medical education. At the same time, more quantitative data on global health in medical education should be collected. Teaching the global aspects of medicine and understanding medical resources and care in a developing country will prepare future physicians to have a more complete understanding of health and medicine and will encourage them to pursue primary care specialties and to serve in resource-poor settings. This, in turn, will strengthen our health care system.

Global health in medical education: a call for more training and opportunities.

Abstract

Worldwide increases in global migration and trade have been making communicable diseases a concern throughout the world and have highlighted the connections in health and medicine among and between continents. Physicians in developed countries are now expected to have a broader knowledge of tropical disease and newly emerging infections, while being culturally sensitive to the increasing number of international travelers and ethnic minority populations. Exposing medical students to these global health issues encourages students to enter primary care medicine, obtain public health degrees, and practice medicine among the poor and ethnic minorities. In addition, medical students who have completed an international clinical rotation often report a greater ability to recognize disease presentations, more comprehensive physical exam skills with less reliance on expensive imaging, and greater cultural sensitivity. American medical students have become increasingly more interested and active in global health, but medical schools have been slow to respond. The authors review the evidence supporting the benefits of promoting more global health teaching and opportunities among medical students. Finally, the authors suggest several steps that medical schools can take to meet the growing global health interest of medical students, which will make them better physicians and strengthen our medical system.

Evidence-based public health: what does it offer developing countries?

The global burden of disease and illness is primarily situated in developing countries. As developing countries have limited resources, it is particularly important to invest in public health and health promotion strategies that are effective. Systematic reviews are central to evidence-based public health and health promotion practice and policy. This paper discusses issues surrounding the relevance of evidence-based public health and systematic reviews to the health of developing countries. It argues that there is a lack of systematic reviews relevant to the health priorities of developing countries; many interventions reviewed can not be implemented in resource-poor situations; and, a limited amount of primary research is conducted in developing countries.

The public health evidence base urgently needs strengthening, with dedicated effort towards increasing the relevance of primary evidence and systematic reviews.



Background

However, a key concern for those promoting evidence-based public health, and relying on systematic reviews of research to inform decisions about public health interventions, either from the Cochrane Library or those published elsewhere, is that often the available reviews are unable to provide guidance on effective interventions,6 particularly in relation to the health of developing countries.

Some reasons why systematic reviews have had a limited role in providing evidence relevant to developing countries are: 

(1) there is a lack of systematic reviews that are relevant to the health priorities of developing countries; 

(2) many interventions that have been reviewed and shown to be effective can not be implemented in resource–poor situations; 

(3) there is a limited amount of primary research conducted in developing countries as compared to high–income countries.

These issues limit the usefulness of currently available systematic reviews (and primary research) for decision-makers in developing countries. This paper discusses the above challenges, critically considering the role of evidence–based public health and the relevance of systematic reviews within the context of international health, and makes some suggestions as to how these challenges could be overcome.

Currently available systematic reviews do not reflect developing world priorities. Two-fifths of the world’s population live in high mortality developing countries, where relatively few risk factors account for the high rates of disease and injury. The 10 leading factors threatening health globally are underweight, unsafe sex, high blood pressure, tobacco consumption, alcohol consumption, unsafe water, sanitation and hygiene, iron deficiency, indoor smoke from solid fuels, high cholesterol, and obesity.9 In high mortality developing regions, underweight, unsafe water, sanitation and hygiene, and indoor smoke from solid fuels are leading risks to health. Approximately one–sixth of the entire disease burden in these regions is attributable to underweight and micronutrient deficiencies, unsafe sex accounts for around one-tenth of all disease burden, and unsafe water for a further 4–5 per cent of the burden (Fig. 1).9







A possible reason for the lack of review questions addressing priority issues for developing areas is that there is a discernible difference in output between reviewers from developing countries (or with significant developing world experience) and those from developed countries. A survey of the place of residence of Cochrane reviewers from 1997 to 2003 suggests that although the number of reviewers from developing countries has increased since the beginning of the period, the relative proportion of reviewers from developing countries compared to developed countries has declined from 16 per cent in 1997 to 8 per cent in 2003 (Fig. 2). This highlights the need for a programme of training and support for reviewers from developing countries.

Conclusion

Evidence-based public health promotes use of the best available evidence on the effectiveness and lack of effectiveness of interventions. Significant efforts are required, however, to increase the relevance of systematic reviews to public health priorities in developing countries and resource–poor areas: the questions and scope need to consider heterogeneous contexts to ensure relevance, and difficult decisions need to be made about the availability and adequacy of the primary research to address the question. Given the social complexity of human development, and the inter-sections amongst different development goals, there is no question that gains in developing country public health are unlikely to emerge from systematic reviews alone, but will require decisions about inter-sectoral collaboration and social policy initiatives.4 However, evidence around intervention effectiveness has an important role to play in efforts to improve developing country health, as it can distinguish what is a worthwhile intervention and what are the relative costs, benefits and anticipated outcomes. The public health evidence base urgently needs strengthening, and requires international collaboration and dedicated effort towards understanding and improving the factors which influence the relevance of primary evidence and systematic reviews to international health.

“We Learn From Them, They Learn From Us”: Global Health Experiences and Host Perceptions of Visiting Health Care Professionals

목적 : 선진국(developed countries)의 보건의료 전문직이 개발도상지역에 방문하여 가르치거나 교육을 하는 사례가 많아지고 있다. 이 프로젝트는 개발도상지역의 보건의료 전문직이 그 지역에 교육을 하기 위해서 방문한 사람들에 대한 인식을 조사하기 위해서 수행되었다.


방법 : 2011년 7월, Namibia 의과대학의 9명의 보건의료 전문직과 반구조화(semistructured)된 면접을 하였다. 혜택(benefits), 피해(harms), 윤리적 인상(ethical impressions)등에 관한 질문을 하였다. 인터뷰는 녹화되고 녹취록을 작성하여 inductive, iterative approach로 질적 분석을 수행하였다.


결과 : 인터뷰를 분석한 결과 세 가지 주된 주제가 확인되었다. (1)문화, 맥락(context), 그리고 관심(concern), (2)기대, 의도, 의사소통의 문제 (3)파트너쉽, 지식을 공유하고 얻으려는 열망


결론 : 인터뷰 참가자들의 말에 따르면 장기적으로 지속가능한 관계를 맺고 적절한 태도를 갖추기 위해서는 문화와 환경적 맥락에 대한 정보를 사전에 파악하여 수요조사(needs assessment)를 방문 전에 실시하는 것이 반드시 필요했다. 이러한 것들은 국제적 교육 협력이 상호간 모두 이익이 되려면 어떻게 만들어져야 하는지에 대한 귀중한 통찰을 제시해준다.





Analysis


The interviews were transcribed by one of the authors (C.K.), and we analyzed the data using an inductive, iterative approach, as described by Auerbach and Silverstein.10 


This method is designed to allow the researcher to generate hypotheses from the data rather than to impose existing theories. The principal investigator (C.K.) repeatedly read the interview transcripts and coded them by hand for relevant text and repeating ideas, grouping these into themes that represented narratives shared by groups of participants. The coinvestigator (C.C.) verified the coding and themes and had no disagreements with the initial classifications. In the last interviews, few new ideas or themes relevant to the research questions were apparent, and no more interviews were carried out.

Abstract

PURPOSE:

It is increasingly common for health care professionals from developed countries to travel to developing regions of the world to learn or teach. This project aimed to describe the perceptions held by health care professionals in a developing region toward those who visit their communities to learn or teach.

METHOD:

Semistructured interviews were conducted in July, 2011, with nine health care professionals from the University of Namibia School of Medicine. Questions revolved around participants' perceptions of benefits, harms, and ethical impressions of a health care professional visiting from a developed country. Interviews were tape-recorded, transcribed, and analyzed qualitatively using an inductive, iterative approach.

RESULTS:

The interview analysis identified three main narratives that shaped participant perceptions of visits: (1) culture, context, and concern, (2) expectations, intentions, and miscommunications, and (3) partnership and the desire to share and gain knowledge.

CONCLUSIONS:

Participants' comments supported actively seeking out information regarding cultural and environmental context before visiting, completing a needs assessment to ensure that activities are needed and relevant, attempting to formulate long-term sustainable relationships, and traveling with the appropriate attitude. These themes provide valuable insight into how international educational collaborations can be created in order to be mutually beneficial.

보건의료분야에 종사하는 여성들에게는 직장과 가정의 균형을 맞추는 일이란, 그로 인해서 생기는 승진속도 저하나 자기분야에서의 뒤쳐짐을 고려하면 무척 어려운 일이다. 이 논문에서 저자들은 교수들의 일과 일상생활의 균형을 향상시키기 위한 혁신적인 개입법을 도입한 UC Davis 의과대학의 경험을 소개한다. 이는 주로 '커리어 융통성 정책(flexible career policies)'에 대한 지식, 인식, 접근을 향상시키는 것을 기반으로 하였다.


그 후 두 개의 교수대상 설문조사의 결과를 요약하였다. 하나는 개입방법을 사용하기 전에 수행된 것이고, 다른 하나는 3년짜리 개입방법이 시행된 후 일년이 지났을 때의 설문결과이다. 두 개의 설문은 모두 교수들의 커리어 융통성 정책을 얼마나 활용하고, 활용할 계획이 있는지, 가능한 옵션에 대해서 얼마나 알고 있는지, 실제적인 장애물은 무엇이며 관련된 교육활동에 참여하는지, 정책에 대한 태도가 변하였는지, 느껴지는 장애물이 줄었는지 등을 설문하였다.


결과는 대부분 여성 교수들이나 50세 이하 교수들에서 두드러졌다(pronounced). 저자들은 비단 의학분야 뿐만 아니라 다른 분야에서도 향후 유사한 연구를 위해 더 논의해야 할 점을 제시하였다. 마지막으로 저자들은 유연한 커리어 정책만으로는 여자 교수들이 줄어드는 것을 저지할 수 없고, 이러한 정책이 기관의 문화에 완전히 스며들어야 한다고 말하고 있다.

Abstract

The challenges of balancing a career and family life disproportionately affect women in academic health sciences and medicine, contributing to their slower career advancement and/or their attrition from academia. In this article, the authors first describe their experiences at the University ofCaliforniaDavisSchool of Medicine developing and implementing an innovative accelerator intervention designed to promote faculty work-life balance by improving knowledgeawareness, and access to comprehensive flexible career policies. They then summarize the results of two faculty surveys--one conducted before the implementation of their intervention and the second conducted one year into their three-year intervention--designed to assess faculty's use and intention to use the flexible career policies, their awareness of available options, barriers to their use of the policies, and their career satisfaction. The authors found that the intervention significantly increased awareness of the policies and attendance at related educational activities, improved attitudes toward the policies, and decreased perceived barriers to use. These results, however, were most pronounced for female faculty and faculty under the age of 50. The authors next discuss areas for future research on faculty use of flexible career policies and offer recommendations for other institutions of higher education--not just those in academic medicine--interested in implementing a similar intervention. They conclude that having flexible career policies alone is not enough to stem the attrition of female faculty. Such policies must be fully integrated into an institution's culture such that faculty are both aware of them and willing to use them.

남인도 지역사회에서 교육 후 조충증/낭미충증(taeniasis/cysticercosis)에 대한 지식과 수행능력 변화

낭미충증의 풍토평(endemic for cysticercosis)이 있는 남부 인도지역의 시골 지역사회에서 조충증/낭미충증(taeniasis/cysticercosis)에 대한 교육 프로그램을 초등학생을 대상으로 시행하였다. 기초조사를 시행했을 때 임의로 선택된 831명의 참가자들은 조충증과 신경낭미충증에 대한 지식이 매우 부족한 상태였다. 또한 적절한 위생시설이나 개념도 부족했다.


이 마을 사람들에게 갈고리촌충의 생활주기, 조충증이나 낭미충증의 전파경로, 예방에 대한 보건교육이 시행되었다. 교육 6개월 후 수행한 시험에서 1060명의 참여자가 전반적으로 46%의 지식과 수행능력 향상을 보였다.


조충증/낭미충증의 전파에 대한 인식수준도 3배 가까이 향상되었으며, 식사 전과 화장실 다녀온 후에 비누로 손을 씻는다는 응답은 각각 4.8배, 3.6배 증가하였다. 조충증/낭미충증의 예방에 대한 교육은 지역사회에서 지식와 수행능력을 증진시키고 자가보고를 통한 진단을 높이는데 효과가 있다.

Abstract

A health education programme for taeniasis/cysticercosis was implemented and evaluated among schoolchildren and the general community in a rural block in southern India, an area that is endemic for cysticercosis. The baseline survey among 831 participants from three randomly selected villages showed poor knowledge regarding the spread of taeniasis and neurocysticercosis. There was also a lack of adequate hygiene and sanitation practices. Health education was given in these villages and in the schools located in these villages regarding the lifecycle of the pork tapeworm, spread of taeniasis and cysticercosis, and prevention of these conditions. The post-intervention test conducted 6 months later among 1060 participants revealed a 46% increase in the overall score of knowledge and practices. Awareness about the mode of spread of taeniasis and cysticercosis improved by almost 3 times and the reported practice of washing hands with soap and water before eating improved by 4.8 times and after using the toilet by 3.6 times. One person who reported the passage of tapeworm segments was confirmed to be a carrier of Taenia solium and was treated. The health education given on prevention of taeniasis and cysticercosis was useful in improving the knowledge and practices of the community and also in diagnosing taeniasis through self-reporting.

배경 : 지난 몇 년간 BRAC(Bangladesh Rural Advancement Committee)은 방글라데시의 국가적 결핵 관리(control) 노력을 지원해왔으며, 특히 지역사회의 보건의료 인력의 훈련에 집중해왔다. 이 연구는 지역사회 기반 결핵 훈련 프로그램과 같은 지역에서의 결핵 사례간 상관관계를 보고자 하였다.


방법 : 이 연구는 후향적 단면 연구(cross-sectional retrospective study)로서 객담(가래) 등록자(registers)와 BRAC의 훈련보고서를 바탕으로 하였다.


결과 : 2005년과 2010년 사이에 536회의 훈련 과정을 통해 9037명의 사람들을 훈련시켰다. 검사를 받는 환자의 숫자는 훈련 전 8211명(2004년)에서 훈련 시작시점에서 10961명(2005년)으로 증가하였고, 동시에 도말검사에서 결핵 양성으로 나온 사람도 7.1%에서 11.2%로 증가하였다. 그 후에 결핵으로 의심되거나 진단된 사람의 숫자든 2010년까지도 비슷하게 유지되었다. 환자를 의뢰해온 가장 중요한 소스는 지역사회 건강 자원봉사자와 자기보고 환자들이었고 이들의 전체 환자의 58%를 차지하였다.


결론 : 이 operational research에서 결핵 사례는 초반에 증가하나가, 나중에는 포화(plateau)상태에 이르렀다. 이러한 현상을 설명하기 위한 추가의 연구가 필요해 보인다.

Abstract

Background For several years, BRAC (previously known as the Bangladesh Rural Advancement Committee) has been assisting with national TB control efforts in Bangladesh and has especially focused on training of community healthcare personnel. This study attempts to determine whether there is any association between a community-based TB training programme in peri-urban Dhaka and TB case finding within the same catchment area.

Methods This was a cross-sectional retrospective study using laboratory sputum registers and annual BRAC training reports.

Results Between 2005 and 2010, there were 536 training activities for community healthcare providers with 9037 people trained. Numbers of patients attending laboratories with suspected TB increased from 8211 in 2004 (before training) to 10 961 in 2005 (start of training) with the proportion diagnosed with smear-positive TB increasing from 7.1% to 11.2%. Thereafter, the numbers with suspected and diagnosed TB remained similar up to 2010. The most important sources of referral of patients for investigation were community health volunteers and self-referring patients accounting for 58% of all patients.

Conclusion In this operational research study in peri-urban Dhaka, there was an initial increase in TB case finding with numbers then reaching a plateau despite continued training activities. Further prospective evaluation is required to understand these phenomena.

이 연구는 HIV-STI(성 매개 감염) 위험을 낮추기 위한 개입법(intervention)과 관련된  훈련 프로그램을 개발하고 수행하기 위한 목적으로 시행되었다. 또한 훈련 프로그램을 통해서 피훈련자의 지식, 태도, (자가보고)역량, 자신감의 변화가 어느 정도 되는가를 살펴보고자 하였다.


우리는 한 대학에 HIV와 STI 예방을 위한 훈련 프로그램과 자료를 개발하여 배포하였고, 훈련은 Integrated HIV-STI Risk Reduction Program이라는 더 넓은 범위의 프로젝트에 참여하기로 되어 있는 facilitator 후보들을 대상으로 시행하였다.


열 명의 facilitator들이 훈련 전-후의 설문을 작성하였고, 훈련 프로그램의 영향을 분석을 위해서는 SPSS를 이용하여 Wilcoxon signed rank test를 시행하였다. 전반적으로 HIV와 관련한 피훈련자의 수행능력, STI 지식, 태도, 스티그마 점수(stigma score)가 크게 향상되었다.


훈련을 마친 후 HIV와 STI지식의 중앙값은 각각 22.0에서 30.5, 8.0에서 9.5로 크게 상승하였으며, HIV와 STI 예방에 대한 긍정적인 태도는 39.0에서 57.0으로 상승하였다. 또한 프로그램을 마친 뒤, 80~100%의 피훈련자가 성건강 개입 활동(sexual health intervention activities)의 수행에 대해 스스로 능력을 갖췄고 자신감이 생겼다고 응답하였다. 


이번 예비 연구로부터 성건강개입(sexual health intervention)에서, 개별 요구에 맞춘 현지 교육은 HIV-STI 예방과 관련한 지식, 태도, 수행능력에 향상 효과가 있음을 알 수 있다.










Peer 교육방법은 HIV/AIDS를 해결하기 위한 전략 중에서 전세계적으로 가장 많이 사용되는 방법 중 하나이다. 통상적으로 타겟 그룹의 멤버를 훈련하고 지원하여 그 그룹에 지식, 태도, 믿음(belief), 행동 등을 개개인 수준에서 변화시키고자 하는 것이다. 사회의 규범을 변화시키거나 정책이나 프로그램에 영향을 줄 수 있는 총체적인 행동의 변화를 유도하여 사회적 수준의 변화도 일으킬 수 있다.


Peer education is one of the most widely used strategies throughout the world to address the HIV/AIDS pandemic.14,15 It typically involves training and supporting members of a given group to effect change among members of that group, and often used to effect changes in knowledge, attitudes, beliefs and behaviors at the individual level.16–18 It can also create change at the group or societal level by modifying norms and stimulating collective action that contributes to changes in policies and programs.19 In Nigeria, where young people now make up the main pool sustaining the epidemic, the role of effective youth specific educational and behavioral change interventions aimed at preventing HIV and other STIs cannot be overemphasized in curbing the escalating HIV epidemic.

Summary

This study aimed to develop and implement a customized training program related to the delivery of an integrated human immunodeficiency virus – sexually transmitted infections (HIV-STI) risk reduction intervention for peer-facilitators and to evaluate its immediate outcome including changes in trainee knowledge, attitudes, and self-reported competence and confidence. We developed and delivered a structured training program and materials about HIV and STI prevention in a university setting. The training was offered to candidate facilitators who were planned to be involved in a larger project, known as Integrated HIV-STI Risk Reduction Program. Ten candidate facilitators participated in the training program and completed both the pretest and posttest survey questionnaire. The data were analyzed using SPSS version 17.0 software package and Wilcoxon signed rank test was applied to assess the impact of the training program. Overall, the trainees’ performance in HIV-related and STI knowledge, attitude and stigma scores had significantly increased compared to the baseline. The median scores for HIV and STI knowledge after the training significantly increased from 22.0 to 30.5 (p=0.007) and 8.0 to 9.5 (p=0.005), respectively, whereas the median score on the positive attitude towards HIV and STI prevention rose from 39.0 to 57.0 (p=0.011). Upon completion of the program, 80–100% of the trainees believed that they were competent and confident in performing most of the designed sexual health intervention activities. This preliminary study suggests that a customized on-site training program on sexual health intervention could significantly improve their knowledge, attitude and practice related to HIV-STI prevention.

Mentorship is a key component of professional development in the field of academic medicine


비공식적(informal)멘토링이 있고, 공식적(formal) 멘토링이 있다.

Informal mentoring occurs spontaneously, as mentors and mentees form a successful relationship built on shared interests and interpersonal chemistry.1 Formal mentoring develops around a systematic infrastructure that aims to replicate the effect of informal mentoring.1


멘토링을 활용하는 정도는 사람마다, 기관마다 차이가 있어서 19%와 84%까지로 다양하다. 1966년부터 2002년까지의 연구를 분석한 리뷰 논문에서 의과대학생과 의사들을 위한 멘토링 프로그램은 수련 기간이 종료되고부터는 점차 줄어든다고 보고된 바 있다.

The prevalence of mentoring in academic medicine varies: Between 19% and 84% of clinical faculty members reported currently working with a mentor in a recent review.2 A review of the literature from 1966 through 2002 that describes mentoring programs for medical students and doctors suggests that mentoring becomes less common once formal training is complete.3

Specifically, our aims were (1) to identify articles published since the last review on this topic (i.e., articles published between 2000 and 2010) that describe models for mentoring programs for physicians in practice, (2) to describe the objectives and core components of these programs, and (3) to summarize the relative benefits of each model and their elements

Seven mentoring models were described in the reviewed articles: dyad, peer, facilitated peer, speed, functional, group, and distance mentoring. The traditional dyad, pairing a mentee with a more senior or more experienced mentor, was most common and was the only model in place for nine programs.5–7,9,10,17,18

Variations of the dyad model, functional mentoring and speed mentoring, were also described. 

The functional mentor was paired with a mentee to provide guidance for a specific project.15

Speed mentoring was a one-time event with mentees and mentors paired for 10-minute periodsto initiate mentoring relationships.14



피어 멘토링 모델만을 사용한 논문도 있다. 이 중 한 연구에 참여했던 멘티들은 Top-down형식의 멘토링이 아니어서 좋았다고 응답했으며, 다른 연구에서는 피어 멘토링에는 투자해야 할 것은 시간과 헌신적인 자세 뿐이라는 사실에 만족해했다.

Two articles described the use of only the peer mentor model, through which groups of individuals similar in age, experience, and rank mentor one another.11,19 Mentees in one of these studies favored that program development did not occur in a topdown fashion.19 The other group felt that peer mentoring is especially beneficial in areas with fewer resources because peer support requires nothing more than time and commitment.11


로컬 멘토가 부족한 점을 다른 기관의 시니어 멘토와의 협조를 통해서, 즉 피어 멘토링과 원거리 멘토링을 활용하여 극복한 사례도 있다. 또 다른 연구에서는 일대일, 피어, 원거리 멘토링을 혼합한 방법을 사용하기도 한다.

One program overcame the scarcity of local mentors by collaborating with a senior mentor at another institution, a combination of peer and distance mentoring.16 Another employed dyadic, peer, and distance mentoring with the mentees supported by a peer mentor (i.e., a colleague close in academic rank to the mentee), a local mentor (i.e., a medical faculty mentor from the same institution as the mentee), and a distance mentor (i.e., a mentor from outside the mentee’s institution).12


Program objectives

멘토링 프로그램의 목적은 다양했는데, 특정한 목적에 따라 디자인 된 것도 있었고, 좀 더 종합적인 목적에 따라 디자인 된 것도 있었다. 가장 흔한 목적은 다음과 같다.

Program objectives varied widely. Some programs were designed to meet specific needs, and others were designed to be more comprehensive. The most common global objectives of mentoring programs were (1) professional or career development,5,7–9,11,12,16,17 (2) academic success,6,8,10,13 (3) networking,6,7,10,14,16,19,20 and (4) faculty retention.10,12

We undertook this review to determine the components that build successful, formal mentoring programs. The term “formal” in this context indicates that the articles described a recognized infrastructure for mentoring

Although the 16 articles varied in the degree to which they described systematic components, we identified seven key components across multiple programs as detailed below.


Mentor preparation

As an element of organizational readiness, several programs addressed mentor preparation.5,9,10,12,18 Llewellen-Williams and colleagues12 developed a Mentor Readiness Inventory and found that mentors desired both retraining to enhance their teaching skills and instruction on, specifically, how to mentor.

Several programs were overseen by a team or committee.10,15,17,18 Committee members included faculty15,17 and “senior department managers.”17 Committee responsibilities included pairing mentees and mentors,15,18 program oversight and design,17 program monitoring and intervention as needed,15 evaluation and data interpretation,17 and assurance of program effectiveness.17

A minority of reviewed programs were structured around a single activity, such as speed mentoring14 or group mentoring sessions during a national professional conference.20

The funding for mentoring programs came from both external5–10 and internal10–12,19sources.



Evaluation and outcomes

Of the programs that evaluated results, most gathered data from mentees6–8,13–19; fewer also collected data from mentors.6,10,14,15,17,18,20 Data collection was predominantly by survey,7,8,10,13–18,20 though program leaders also used participant interviews6,8 and focus groups6,18,19 as means of collecting data


Barriers to program development

멘토링의 장애물에 대해서 언급한 논문의 저자들은 거의 없었다. 정해진 시간이 없는 멘토링 프로그램에서는 대부분의 멘티들이 멘토의 시간 여유 부족을 가장 큰 문제로 꼽았다. 정해진 시간이 없는 것은 프로그램 구성이나 멘토 모집에 장애물로 작용한다.

The authors of the articles we reviewed seldom mentioned barriers to developing mentoring programs. In a mentoring program with no protected time, most mentees felt that mentors’ lack of time was detrimental to the program.18 Lack of protected time was also identified as a barrier both to program organization6 and to mentor recruitment.20

In our review the dyad mentoring relationship remains, as it was in the last 35 years of the 20th century, the most frequently described model. Allowing mentees to choose mentors in the traditional dyad model is highly valued.15,21 In the business world, this practice of “managing up” has encouraged mentees to take control of the mentoring relationship which helps ensure mentee success.21

Our review indicates that mentors are now trained to perform their role in some programs; such training was completely absent according to the last review. A weakness noted previously2,3 and persisting through the articles we reviewed is that reported results remain mostly descriptive, local, subjective, unvalidated, and without standardized evaluative metrics, such that no conclusions can be made regarding the effect of individual program components on mentoring outcomes.


Implications for medical practice

멘토의 수가 제한적인 상황에서는 전통적인 일대일 방식보다는 peer 또는 facilitated 모델이 효과적일 수 있다. 멘토링 프로그램에 대한 적절한 지원은 성공을 위해 필수적이며, 시간적으로 제한되거나 정해진 시간이 없는 상황은 멘토링 프로그램에 장애물로 작용한다. 멘토링 프로그램을 위한 정해진 시간이 있는 프로그램에 참여했을 때, 그 참가자들은 기관(조직)이 멘토링을 제대로 하고 있다고 받아들인다. 또한 계약서를 작성하는 것은 멘토링 관계를 좀더 확고하게 만들어서 mission statement를 사용하거나 signed agreement를 활용하는 것이 멘토링 관계에 책임감을 높이는데 도움이 된다.

In settings with limited mentors, peer and facilitated models help extend available resources and benefit more mentees than would be possible with the traditional dyad model. Adequate support for the mentoring program is a key ingredient to success because sustaining mentoring activities without support is difficult.10 Limited or unprotected time was often cited as a barrier to program development. Participants with protected time viewed this commitment from their institutions as a sign of acceptance for mentoring.15 Lastly, although other reviews have noted that contracts may make the mentoring relationship inflexible,22 the use of mission statements to set boundaries and of signed agreements to enforce accountability to mentoring relationships may be helpful.5–7,10,11,13,16


Going forward

Program evaluation for the most part, however, remains largely subjective or focused on specific, local program aims and short-term results. Standardized metrics would facilitate cross-institution research and enhance generalizability. To this end, Berk and colleagues25 developed two questionnaires to comprehensively assess the mentoring relationship by evaluating behavioral characteristics of the mentor as well as the characteristics and outcomes of the mentoring relationship

Additionally, given the likely longitudinal effects of mentoring on individuals’ careers, examining and reporting longterm outcomes is essential.

Abstract

PURPOSE:

Mentoring is vital to professional development in the field of medicine, influencing career choice and faculty retention; thus, the authors reviewed mentoring programs for physicians and aimed to identify key components that contribute to these programs' success.

METHOD:

The authors searched the MEDLINE, EMBASE, and Scopus databases for articles from January 2000 through May 2011 that describedmentoring programs for practicing physicians. The authors reviewed 16 articles, describing 18 programs, extracting program objectives, components, and outcomes. They synthesized findings to determine key elements of successful programs.

RESULTS:

All of the programs described in the articles focused on academic physicians. The authors identified seven mentoring models: dyad, peer, facilitated peer, speed, functional, group, and distance. The dyad model was most common. The authors identified seven potential components of a formal mentoring program: mentor preparation, planning committees, mentor-mentee contracts, mentor-mentee pairing, mentoring activities, formal curricula, and program funding. Of these, the formation of mentor-mentee pairs received the most attention in published reports. Mentees favored choosing their own mentorsmentors and mentees alike valued protected time. One barrier to program development was limited resources. Written agreements were important to set limits and encourage accountability to the mentoring relationship. Program evaluation was primarily subjective, using locally developed surveys. No programs reported long-term results.

CONCLUSIONS:

The authors identified key program elements that could contribute to successful physician mentoring. Future research might further clarify the use of these elements and employ standardized evaluation methods to determine the long-term effects of mentoring.
