일본 의료의 현변화
Current changes of health care in Japan

의료 교육이 의료의 지역적 및 세계적 변화에 대응함에 따라(Gibbs 및 McLean 2011), 의과대학은 지역사회에 봉사할 의료 전문가를 양성해야 할 사회적 책임을 지고 있습니다. 일본의 의료·의료 교육에 대해 기술한 마지막 논문(오니시·요시다 2004년·고즈 2006년·스즈키 외 2008년)으로부터 근 10년, 일본의 의학교육은 과거 수십년간 사회·의료 개혁의 필요성(2014년 Ban)과 그 이하에 관한 큰 전환점을 맞이하고 있다. 지난 수십 년간 '세계화'의 영향 현재의 일본 의학 교육의 변화와 과제에 대처하기 위해, 박스 1과 같이, 헬스케어의 큰 변화를 몇 개 선택했습니다.

As medical education responds to regional and global changes in healthcare (Gibbs and McLean 2011), medical schools have a social responsibility to train health care professionals to serve local communities. It has been almost a decade since the last paper describing health care and medical education in Japan (Onishi and Yoshida 2004; Kozu 2006; Suzuki et al. 2008), and medical education in Japan is now approaching a major turning point, related to the need for the reformation of society and healthcare in the past few decades (Ban 2014), and under the influence of “globalization” for the past decades. To address the current change and challenge of medical education in Japan, we have selected some major changes in healthcare as shown in Box 1.

 

일본의 번역주의
Japan’s “translationism”

국제화에 대한 일본인의 관점은 일본 의학 교육의 현재 상태에 기초하고 있다. 2010년 일본의 철학자이자 베스트셀러 중 하나인 '일본의 주변국적 인식에 관한 논의'에서 우치다(2009)는 국제화에 대한 일본인의 공통된 견해를 논했다. 우치다씨는, 일본인은 주로 [주변적 상황]에 살고 있다는 인식에 근거해, 세계의 중심이 세계 어딘가에 있다고 생각하는 경향이 있다고 주장했다. 그 때문에, 우치다씨는, 통상, 일본인은 새로운 글로벌 동향이나 컨셉을 오픈 마인드(비판 없이)로 받아 들여, 국제 표준을 따라잡아, 서서히 해석·실행해, 최종적으로 일본 국내에 도입한다고 생각하고 있다. 일본의 다른 철학자인 마루야마와 가토(1998년)는 근대 일본이 19세기 개국 이후 외국 문화를 수입함으로써 더 큰 변혁을 이루었기 때문에 "수입"의 중요성을 언급했다. 해외에서 개념을 수입하는 것은 사실 다른 언어에 대한 이해를 높여야 하며, 19세기 일본 정부는 모든 외국어를 일본어로 번역해야 한다는 번역주의를 채택했다. 그 결정 과정에서 일부 사람들은 영어가 일본의 공용어 중 하나로 받아들여져야 한다고 주장했다. 반면에, 다른 사람들은 영어를 이해하지 못하는 일반인들은 중요하고 학술적인 토론에서 제외되고 사회적 불평등을 야기할 수 있기 때문에 제안된 정책에 동의하지 않았다. 이러한 번역주의는 세계화의 쓰나미에 대한 방파제 역할을 하고 문화적 독립을 유지할 수도 있다. 실제로 번역주의의 적응에 의해, 일본어를 사용하는 것에 의해서, 일본의 전통 문화에 비해, 서서히 외국의 개념이나 제도, 문화를 내면화할 수 있을지도 모른다(마루야마, 카토 1998). 한편, 일본 제품의 외국어로의 번역은 매우 한정되어 있어, 이 기사에 기재되어 있는 레퍼런스의 상당수는 일본어로 되어 있다.
The Japanese perspectives toward internationalization are fundamental to the status quo of Japanese medical education. In “Nihon Henkyo Ron (Discussion on the Japan’s perception of peripheral nationality)”, which was written by a Japanese philosopher and one of the best-selling book in 2010, Uchida (2009) discussed the Japanese common perspectives on internationalization. Uchida claimed that the Japanese are based primarily on their perception that they are living in a peripheral situation, and tend to think that the center is located somewhere else in the world. Therefore, Uchida considers that Japanese usually accept new global trends and concepts with an open mindset (without criticisms), to catch up with international standards, interpret and implement them gradually, and finally incorporate them internally into the Japanese context. Maruyama and Kato (1998), other Japanese philosophers, stated the importance of “import” since modern Japan had achieved further transformation by importing foreign cultures after the opening of the country in the nineteenth century. Importing concepts from overseas actually requires a better understanding of another language and in the nineteenth century, the Japanese government adopted “translationism” that every foreign language should be translated into Japanese. In the process of making the decision, some people argued that English should be accepted as one of the official languages in Japan. On the other hand, others disagreed with the policy suggested, because common people who could not understand English would be excluded from the important and scholarly discussions and possibly resulting in social disparity. Such translationism might function as the breakwater against the tsunami of globalization, and maintain cultural independence. In fact, the adaptation of translationism might enable Japanese to internalize foreign concepts, system, and cultures gradually, in comparison with Japanese traditional cultures, by using Japanese language (Maruyama and Kato 1998). On the other hand, translation of Japanese products into foreign languages has been extremely limited, as many of references in this articles are in Japanese.


이 기사에서는, 박스 2와 같이, 매크로, 메소, 마이크로 레벨의 글로벌 동향과 비교해, 우리 나라의 의학 교육의 「번역」의 개념, 현재의 상황, 변화에 초점을 맞추고 있다.

In this article, we focus on some “translated” concepts, current context and changes of medical education in Japan, in comparison with global trends at macro, meso, and micro level as shown in Box 2.

매크로 레벨(국제, 국내 레벨)
Macro level (international, national level)

이 수준에서는 문화보다는 [사회 시스템]이 교육 변화에 영향을 미칩니다. 고령화 고령화와 지역 내 의사 분포의 불균형에 의해 일본의 사회·질병 구조가 변화하고 있다. 실제로 우리나라의 의사 수는 OECD 국가 중 매우 낮은 수준이며(OECD 2009), 도시화와 의사 부정 분포가 공공연히 주장되었다(타니하라 외 2011). 그 결과, 일본인은 다음과 같은 다양한 어프로치에 의해, 지역사회에서 일하는 의사를 늘리려고 하고 있다.
At this level, the social system rather than culture influences the educational change. The social/disease structure in Japan has been changing due to the super-aging population and the ever-increasing uneven regional distribution of doctors. In fact, the number of physicians in Japan is ranked very low among OECD countries (OECD 2009), and urbanization and physician mal-distribution were claimed publicly (Tanihara et al. 2011). As a result, Japanese are trying to increase the physicians working in the community by various approaches as described below.

지역사회에서 인적/사회적 자원을 효과적으로 활용할 수 있는 [1차 진료의사 양성]은 졸업 후 다양한 수준에서 이루어지고 있다(Ban and Fetters 2011). 2014년에는 프로그램의 표준화 및 전문의의 질 보장을 위한 독립된 제3자 조직으로 재조직되어 재기동되어 general practice가 내과, 외과 등 19개 기본 의학 전공분야 중 하나로 자리매김하고 있다. 전국적인 프로그램이 개발 중입니다. 우리나라는 모든 의료기관에 대한 무료접근 체제를 갖추고 있지만, 게이트키퍼로서의 일반의의의 증가는 환자의 의사접근의 재정비와 의료비 절감에 기여할 것이다.

Training of the primary care physicians who can utilize human/social resources effectively in the community is undertaken at various levels after graduation (Ban and Fetters 2011). In 2014, the Japanese Medical Specialty Board was re-organized and re-started as an independent third party organization for standardizing programs and guaranteeing the quality of specialists, and the general practice is finally positioned as one of 19 fundamental medical specialty areas such as internal medicine and surgery and its development of nationwide programs is under development. Although Japan has a free-access system to all health care institutions, the increase in certified general practitioners as gatekeepers would contribute to the reorganization of the patients’ access to the physicians and reduction of medical expenditure.

국제적인 동향에 맞추어, 의학교육의 모든 수준에서 임상 훈련은 서서히 대학병원에서 지역사회로 이동해 왔다(일본 커뮤니티에 근거하는 의학교육협의회 2015). 레지던트 트레이닝에 대해서는, 2004년에 2년간의 대학원 임상 트레이닝 프로그램이 도입되었습니다(스즈키 외 2008년). 이 프로그램에서는 커뮤니티에서 1개월간의 트레이닝을 의무화하고 있으며, 이는 커뮤니티에서 활용할 수 있는 기초적이고 포괄적인 임상역량을 배양하기 위한 노력의 일환이기도 합니다. 다른 나라의 경향과 같이, 커뮤니티 클리닉에서의 커뮤니티-기반 교육과, 일차의료-지향적 교육은 있어서의 교육이 일본의과대학 80곳 중 77곳에서 도입되고 있다(Takamura et al.2015). 또, 학부 교육에서는, 커뮤니티에의 조기 공개를 실시해, 커뮤니티의 고령자, 임산부, 영유아와의 장기적인 만남이, 사회 주체로서의 자각의 폭을 넓히고, 커뮤니티의 기능을 이해하는 데 성공하고 있다.

To fit with the international trends, clinical training has slowly been relocated from the university hospitals to the community at all level of medical education (Japanese Council for Community-based Medical Education 2015). As to residency training, a two year-postgraduate clinical training program was introduced in 2004 (Suzuki et al. 2008); one month training in the community is now mandatory during this program, which is also a part of efforts to cultivate the basic and comprehensive clinical competencies that can be utilized in the community. As with the trend in other countries, community-based education and primary-care-oriented education, within community clinics has been introduced in 77 out of 80 Japanese medical schools (Takamura et al. 2015). Furthermore, early exposure to the community is provided in undergraduate education; longitudinal encounters with elderly people, pregnant mothers, and infants at the community were all successfully implemented for the first year students to understand the function of the community with broadening their self-awareness as social entities (Saiki et al. 2016).

“Chiiki-waku” to resolve the maldistribution of doctors

문부과학성은, 지역사회에 봉사하는 의사를 늘리기 위해서, [3개의 시책]을 실시했습니다. 문부과학성은 2008년에 [의과대학 입학정원을 증원]하기로 하고, 2007년 7625명에서 2016년 9262명(21.5%)으로 전국 의과대학 입학정원을 변경했다. 문부과학성의 [2개 의과대학 신설] 결정과 함께, 특히 농촌 지역 취업을 희망하는 학생을 우대하는 [새로운 입시 제도가 도입]되었습니다. 이 혁신에서는, 현내에 거주하는 지방 학생이나, 현내에서 복무할 학생에 대해서, 일정수의 의과대학이 일정 숫자의 입학 정원을 허용하고 있다. 지원자들은 지역사회에서 일하려는 강한 미래 의지를 가질 것으로 기대된다. 입학사정관리는 학업성취도, 고등학교 추천, 면접을 포함한 시험을 바탕으로 이루어진다. 졸업생들이 특정 기간(보통 9년) 동안 지역사회에서 일한다는 점을 고려할 때, 그들은 지방 정부로부터 장학금을 받을 수 있다. 이 「치이키와쿠」를 통해 의대에 입학하는 학생이 계속 증가해, 약 1400명(2016년 문부과학성)의 의과대학이 71개교 이상 있다. 오구치 외 연구진(2015년)은, 「치이키와쿠」의 학생은, 학년 내내 지역·농촌의학에 대해 긍정적인 이미지를 가지고 있어, 그 중에서도 1차 진료나 소아과가 인기가 있는 것을 분명히 했다. [지역틀 졸업자]의 83%가 같은 도도부현에 있는 레지던트 프로그램을 선택하고 있는 것에 비해, [비-지역틀 졸업자]는 45.3%만이 선택하고 있다(오구치 외 2015). 이 지역틀 입학 제도는, 지역·농촌의 장래의 의사 양성에 효과적이라고 생각됩니다.

The Ministry of Education, Culture, Sports, Science & Technology of Japan (MEXT) (http://www.mext.go.jp/en/) had implemented three policies to increase the physicians serving at community. MEXT decided to increase the admission quota of medical schools in 2008, and the nationwide medical student enrollment changed from 7625 in 2007 to 9262 in 2016 (21.5% increase). In addition to MEXT’s decision to establish two new medical schools, a new entrance examination system was introduced that specifically favored students who wished to work in community rural areas. In this innovation, medical schools are allowed to have a certain number of seats for local students living in the prefecture where the medical school exists or any students who will serve in the prefecture community. The applicants are expected to have a strong future will for working in the community. The admission decisions are made based on the examination, including the academic performance, recommendation of high schools and interviews. They can receive a scholarship from the local government, given that the graduates work in the community for an given period; usually nine years. The number of students admitted to the medical school via this “Chiiki-waku” has continued to increase and there are now over 71 medical schools, that accept about 1400 such medical students (MEXT 2016). Oguchi et al. (2015) clarified that students of “Chiiki-waku” had a positive image to regional/rural medicine throughout the period of academic years, and primary care and pediatrics are more popular among them. Eighty-three percent of the regional frame graduates have selected residency program located in the same prefecture of their graduation, compared to 45.3% of the non-regional graduates (Oguchi et al. 2015). This regional frame entrance system seems to be effective for increasing the future doctors in the local/rural community.

JACME의 설립
Establishment of JACME

일본의 의과대학에서는, 「일본의 세계 동향에의 민감성」과「일본의 헨쿄론」(우치다 2009)으로 설명되고 있는 미국에의 역사적 의존성」의 2가지 요인에 의해서, 국제적 인정이 매우 중요한 것으로 인식되고 있다. 또 다른 이유는 일본 의대의 보수적인 상황을 타개하려는 의학교육자들의 동기이다. 2004년에 이미 일반 고등교육의 인증제도가 시작되어 교육기관 전체의 신뢰성을 확보하는 것이 주안점이 되고 있기 때문에, 우리 나라 대학으로서 인정 자체는 새로운 것이 아니다. 그러나 2010년 미국 외대 졸업자 교육위원회(ECFMG)는 "2023년부터 자격증을 취득하려면 ECFMG가 의대 인증을 받아야 한다"(ECFMG 2010)고 발표했다. 그 후, 의학교육을 위한 독자적인 인증 기관을 설립하는 움직임이 급속히 강해지고 있습니다. 문부과학성 및 일본 의과대학연합회(AJMC)가 지원하는 파일럿 QA 프로그램을 마련하여 2013년 4월부터 의과대학에 대한 시험인정을 실시하고 있습니다. 그 결과, 2015년 12월에 JACME(Japan Accreditation Council for Medical Education)가 정식 설립되어 2017년 3월에 세계 의학 교육 연맹으로부터 인정받았다(JACME 2015-2017). JACME가 주관하는 공식 인증 활동은 2017년 4월부터 시행되고 있습니다. 이것은, 우리 나라의 의학교육의 변혁에 영향을 주는 계기가 될 수 있다.

International accreditation is recognized as extremely important by the Japanese leaders in medical schools due to two factors: one is Japan’s sensitivity to global trends and historical dependency on the United States to catch up with international standards, which may be partly explained by the “Nihon Henkyo Ron” (Uchida 2009) as described. Another reason is medical educators’ motivation to break through the conservative situation of medical schools in Japan. Accreditation itself is not something new for Japanese universities since the accreditation system for general higher education had already started in 2004, in which the main focus was to ensure the credibility of educational institutions as a whole. However, in 2010, the US Educational Commission for Foreign Medical Graduates (ECFMG) announced that “ECFMG requires medical school accreditation for international medical school graduates seeking certification beginning in 2023” (ECFMG 2010). Since then, a momentum to build an independent accreditation organization for medical education has rapidly increased. A pilot quality assurance program supported by MEXT and the Association of Japanese Medical Colleges (AJMC) has been organized and has carried out trial accreditation for medical schools since April 2013. As a result, the Japan Accreditation Council for Medical Education (JACME) was formally established on December 2015, and recognized by the World Federation for Medical Education in March 2017 (JACME 2015–2017). Formal accreditation activity, which is organized by JACME, has been implemented since April 2017. This can be an influential trigger of transformation of medical education in Japan.

Meso(조직/조직 수준)
Meso (institutional/organizational level)

이 수준에서, [몇몇 문화적 요소]들이 교육 변화에 영향을 미칠 수 있다. 커리큘럼 개혁, 성과주의 교육, 자기주도적 학습 전략 수립, 효과적인 교사 연수는 세계적으로 주요한 전제입니다. 커리큘럼 개발에는 지역 의료교육기관의 조직 변화(2001년 Genn)가 포함되기 때문에 특정 국가의 커리큘럼 발전에 미치는 문화적 영향도 무시할 수 없다.
At this level, some cultural factors may influence educational changes. As to curriculum reform, outcome-based education, setting self-directed learning strategies and effective teacher training is a major premise globally. Since curriculum development includes organizational change in local medical education institutions (Genn 2001), the cultural influence on the curriculum development in a particular country cannot be ignored.

커리큘럼 개혁
Curriculum reformation

Hofstede 외 연구진(2010)은 일본이 권력 거리에서는 중간, 불확실성 회피에서는 가장 높은 점수를 가지고 있음을 입증했다. 이러한 문화의 특성은 PBL/TBL 및 임상 사무직과 같은 커리큘럼 개혁에 대한 접근 방식에 영향을 미칠 수 있다. Jippes and Majoor(2008)와 Jippes et al.(2013)는 권력 거리 및 불확실성 회피와 같은 Hofstede의 문화적 차원이 PBL 커리큘럼과 통합 커리큘럼의 구현에 부정적인 영향을 미친 국가에서 다음과 같이 제안했다. 스즈키 외 연구진(2008)의 보고에 의하면, 일본의 의대의 90%는 커리큘럼 설계에 PBL 어프로치를 채용하고 있다. 한편, 자격을 갖춘 충분한 튜터 확보나 물적 자원의 가용성 등과 같이 PBL 프로그램을 지속하는 것에 관한 어려움을 보고한 기관도 있다(Okubo 2014). 이러한 어려움으로 인해, 최근 PBL을 사용하는 의과대학 수는 약간 감소 추세를 보이기 시작했다. 또한 구현 방법이 다양하다는 것도 분명해졌다. 예를 들어, PBL은 기초 의학, 임상 과목의 커리큘럼에만 도입되거나 몇 주 동안 다른 커리큘럼에 도입된다(Okubo 2014). 한편, 일본의 일부 의과대학은 PBL에서 팀 베이스 학습(TBL)으로 이행하기 시작했습니다. 의과대학 80곳 중 31곳은 2013년부터 TBL 방식을 채택하고 있지만, 실행하는 방식이 불분명하고, 목적에 대한 적합성은 아직 연구가 필요하다. 임상 사무직은 일본 의료 커리큘럼의 또 다른 약점으로 꼽힌다. 많은 의과대학에서 학생들은 여전히 모든 임상학과를 통해 단기(2주) 교대로 배우고 있지만, 임상실습의 국제 표준은 기본적인 임상 역량을 배우기 위한 핵심 과목의 통합과 집중이다.

Hofstede et al. (2010) demonstrated that Japan has an intermediate score in power distance and highest score in uncertainty avoidance. Such characteristics of culture may influence the approaches to curriculum reformation such as PBL/TBL and clinical clerkship. Jippes and Majoor (2008) and Jippes et al. (2013) suggested that in countries where Hofstede's cultural dimensions such as power distance and uncertainty avoidance negatively influenced the implementation of PBL curriculum and integrated curriculum. As reported by Suzuki et al. (2008), 90% of Japanese medical schools have adopted a PBL approach in the design of their curriculum. On the other hand, some institutions reported the challenges of sustaining a PBL program, such as ensuring the sufficient number of qualified tutors, and the availability of physical resources (Okubo 2014). Due to these challenges, the number of medical schools using PBL has started to show a slight downward trend in recent years. It also has become clear that the ways of implementation are varied. For example, PBL is introduced only for the curriculum in basic medical sciences, clinical subjects only, or introduced for a few weeks in a few other curricula (Okubo 2014). Meanwhile, some medical schools in Japan began to implement, or switch from PBL to team-based learning (TBL); 31 out of 80 medical schools have adopted a TBL approach since 2013 (Okubo 2014), but its implementation is unclear and its fit for purpose still needs further research. Clinical clerkship is considered another weak point of Japan’s medical curriculum. In many medical schools, students still learn through a short (two weeks) rotation through all clinical departments, while international standard of the clerkship is more integration and the concentration of the core subjects to learn basic clinical competencies.


일본에서는 새로운 커리큘럼 학습 전략을 받아들이는 열린 마음이 있을 수 있지만, 실제로는 각 학과의 재량권에 대한 간섭이 있어, 매우 독특하고 간략화된 형태로 되어 가는 경향이 있다. 따라서, 실천 연구(2001년 Genn 2001년)등의 연구를 재검토해, 글로벌한 커리큘럼 경향의 개념을 어떻게 채용해, 우리 나라의 의학 교육에 내재화시키고 있는지를 파악하기 위해서, 커리큘럼 혁신에의 노력과 효과가 필요하다.

In Japan, there may be an open mind to accept new curricula learning strategies; however, in practice, there is an interference with the discretionary rights of each department, and a tendency toward quite unique, shortened and simplified forms. Therefore, efforts and effectiveness for the curriculum innovation are necessary to examine the research such, as action research (Genn 2001), to understand how the concept of the global trends of the curriculum are employed and internalized into Japanese medical education.

 

일본의 교직원 육성
Faculty development in Japan

1969년 일본 의학 교육 협회(JSME)의 설립으로 교수개발이 시작되었다. 일본 최초의 의학교육과·센터는 1972년, 준텐도 대학에 설립되었습니다. 1973년, 3명의 일본인 의료지도자가 시드니에서 세계보건기구(WHO)가 실시하는 지역 교사 양성 프로그램에 참가하고, 1974년에는 후지산 기슭에서 같은 1주간의 「의료 교육자를 위한 의료 교육 워크숍」을 개시했다. 이후 일본에서는 커리큘럼 기획과 교수법을 폭넓게 다루는 워크숍식 세미나가 열렸다. 후생노동성(이하 후생노동성)이 2004년 [2년간의 의무적 레지던트 프로그램]을 개시한 이후, 전국 임상교사 워크숍의 기본 틀이 됐다. 워크숍 참가자는 50세 미만이어야 하며, 프로그램은 16시간 이상 진행되어야 하며, 교육과정 개발 및 피드백 및 직장 기반 평가(문부과학성 2004) 등의 실용적인 방법을 포함해야 합니다. 이 워크숍에는 지금까지 7만 명 이상의 주치의(일본 의사 전체의 약 20%)가 참가해, 인증을 취득했습니다.
Faculty development began in 1969 with the establishment of the Japan Society for Medical Education (JSME). The first department/center of medical education in Japan was established in 1972 in Juntendo University. In 1973, three Japanese medical leaders participated in a Regional Teacher Training Programme provided by the World Health Organization (WHO) in Sydney, and they launched a similar 1-week Japanese program “Medical Education Workshop for Medical Educators” at the foot of Mt. Fuji in 1974. Since then, workshop-style seminars that broadly handle curriculum planning and teaching methods had been held in Japan (Science Council of Japan 1997; Ban 2014). Such workshop have now become the basic framework of the nationwide workshop for clinical teachers in 2004 launched by Ministry of Health, Labour and Welfare of Japan (MHLW) after a compulsory 2-year residency program was started. Participants in the workshop should be less than 50, the program should be run for at least 16 hours, and should contain curriculum development and practical methods such as feedback and workplace-based assessment (MEXT 2004). More than 70,000 attending physicians (about 20% of total Japanese physicians) have participated in this workshop and have been certified so far.

커리큘럼 개혁이 시작된 1990년 이후 현재 의과대학 80곳 중 75곳에 의학교육을 위한 부서나 센터가 존재한다. 600명 이상의 전임 및 시간제 학부가 고용되어 있으며, 비공식 조직 「의학 교육 단위 협회」(Hatao 1999)아래에서 협력하고 있다. 의료 교육을 담당하는 핵심 교직원이 급증함에 따라 교육 전문가 자격증을 취득할 필요성이 높아졌다. 이러한 움직임은 WFME 글로벌 스탠다드 문서에서 의료 교육 전문가를 각 기관에 배치하고 교육 활동을 관리해야 한다는 문구로 가속화되었다(WFME 2015). 현재 일본에서는 외국인 의학교육 석사과정 졸업생이 20명 안팎에 불과하고, 석사과정도 개설되지 않았다. 석사 프로그램 설립이 예상되는 가운데 JSME의 전국 니즈 평가 조사에서 JSME 회원들은 석사 학위보다는 의학 교육의 기본 자격증을 취득하는 것을 선호하는 것으로 나타났다. 이에 따라 2014년 JSME 주도로 학부/대학원/평생 의학교육의 기획, 교육, 경영, 선도, 연구에 능한 [의학교육 전문가]를 양성하기 위한 [전문가 자격 제도]가 구축되었습니다. 지원자는 (1)교육·학습, (2)학습자 평가, (3)교육과정 개발에 관한 3개의 1박2일 과정에 참가하여 교육활동을 분석하여 3개의 교육실천 반영보고서를 작성해야 합니다. 2017년 3월까지 56명의 [의학교육전문가]가 인증되었습니다. 이들은 의학교육이 제도적, 국가적 차원에서 더욱 발전하기 위해 의견 주도적 역할을 할 것으로 기대된다.

Since 1990, when curricula reform commenced, the number of departments/centers for medical education now number 75 out of 80 present medical school (http://www1.gifu-u.ac.jp/∼medc/unit/unit.htm). More than 600 full-time and part-time faculties are employed and they are collaborating under the informal organization “Society for Medical Education Units” (Hatao 1999). With the rapid increase in core faculty members who are responsible for medical education, there was increasing need to acquire certification as experts in education. This movement was accelerated by the statement from the WFME global standards document, that medical education experts are required to be assigned to each institution and to manage educational activities (WFME 2015). Only around 20 Japanese graduates from foreign Masters programs on medical education are now taking a leadership role in Japan, and no Masters program has yet been established. While the establishment of a Masters program is expected, the nation-wide need assessment survey by JSME demonstrated that the members of JSME prefer to obtain a basic certification in medical education rather than a Master degree (Suzuki et al. 2009). According to these results, expert qualification system led by JSME was established in 2014 to nurture medical education experts who are competent in the planning, teaching, managing, leading and studying of undergraduate/postgraduate/lifelong medical education. Applicants are required to participate in three two-day courses on (1) teaching and learning, (2) learner assessment, and (3) curriculum development, with analyzing their educational activities, then they make three educational practice reflective reports for the certification. By March 2017, 56 Medical Education Specialists have been certified. They are expected to take opinion leadership for further development of medical education at both institutional and national level.

대학 레벨에서는, 의학교육에 관한 세미나나 워크숍이 전국에서 개최되고 있습니다. 특히 기후대학 의학교육 개발센터(MEDC)가 주최하는 의료 교육 세미나나 워크숍은, 2001년부터 6000명 이상의 참가자를 받아 들여 새로운 교육 개념과 방법의 발전에 공헌하고 있습니다. 또, 최근, 2개의 의과대학(교토대, 기후대)에서 보건전문직교육 펠로우쉽 프로그램이 개시되고 있다. 두 프로그램 모두 바쁜 임상의들을 위해 온라인 학습과 캠퍼스 학습의 결합으로 설계되었습니다. 이러한 펠로우들은 각 기관의 현지 상황에 대응하여 균형 잡힌 임상 의무와 교육을 보여줄 것으로 기대된다.
At the university level, seminars and workshops on medical education have been held throughout the country, especially Medical Education Seminars and Workshops hosted by Gifu University Medical Education Development Center (MEDC), which as a National Collaboration Centre has accepted more than 6000 participants since 2001 and contributed to the development of new educational concepts and methods (Saiki et al. 2014). Moreover, Fellowship programs on health professions education have recently begun in two medical schools (Kyoto University, Gifu University). Both programs have been designed as a combination of online learning with campus learning for busy clinicians. Those fellows are expected to show their balanced clinical duties and teaching responding to the local context in each institution.

[사회적 구성주의]는 현대 교육 및 학습의 글로벌하고 견고한 이론적 기반 중 하나로 간주됩니다. 이 견해는 지식은 활동을 통해 사회적으로 어떻게 구성되는지에 대한 설명을 제공합니다. 성공적인 학습의 열쇠는 학습자의 적극적인 참여와 교육자의 학습 촉진입니다. 또한 오늘날 성찰은 학습 주기의 중요한 단계로 여겨진다(Kolb 1984). 그러나 학습에 대한 기대와 학습에서 어떤 일이 일어나야 하는지에 대한 개념은 원래 각 국지적 맥락, 즉 상황에 따라 개발되었다. 이 절에서는, 해외로부터의 새로운 교육 개념이나 실천이 어떻게 받아들여져 일본의 의학교육에 동화되고 있는지를 기술한다.
Social constructivism is regarded as one of the global, solid theoretical foundations in contemporary teaching and learning. This view provides an account of how knowledge is socially constructed through activity. Key to successful learning is the learner’s active participation and educator’s facilitation for learning. Additionally, reflection is nowadays seen as an important phase of the learning cycle (Kolb 1984). However, the expectations of learning and concepts about what should happen in learning have been originally developed in each local context, i.e. context-dependent. In this section, how new educational concepts and practices from overseas have been accepted and assimilated into Japanese medical education are described.

일본의 교수법
Japanese approaches to teaching

일본의 의학교육자는 전통적으로 학생의 지식 습득을 바람직한 학습 성과로 중시해 왔으며, [강의 위주의 교습]이 바람직한 교육 전략으로 간주되어 왔다. 한편, PBL, TBL, 플립 교실등의 인터랙티브한 교육 전략을 개발해, 학습자의 자율적인 학습과 문제 해결 능력을 촉진해 왔습니다. 의학 교육의 개혁에 수반해, 일본의 고등 교육에서는, 학습자 중심의 교육 패러다임으로의 이행이 광범위하게 행해지고 있다. 특히, 일본 정부 중앙 교육 협의회(2012년 중앙 교육 협의회)가 대학생의 고등 교육에서의 적극적인 학습을 촉진하는 것의 중요성을 강조했기 때문이다.

The medical educators in Japan have traditionally focused strongly on students’ knowledge acquisition as the desired learning outcome and seen lecture-based teaching as a preferred educational strategy (Onishi and Yoshida 2004). On the other hand, they have also committed to developing the interactive educational strategies over 30 years, such as PBL, TBL and flipped classroom, to promote learners’ autonomous learning and problem-solving skills. Subsequent to the reforms in medical education, a shift to the educational paradigm underpinned by learner-centeredness has been made widely in Japanese higher education, especially as the Central Council for Education of the Japanese Government (Central Council for Education 2012) emphasized the importance of facilitating university students’ active learning in higher education.

일본의 교육 패러다임의 변화 속에서, 「선생님」은 학생 학습의 촉진자 또는 코치로서의 역할을 기대받는다. Saiki et al. (2013)는 일본의 의학 교육자들이 때때로 PBL 튜토리얼에서 튜터/조력자의 역할을 맡는 데 어려움을 겪고 있다고 시사했다. 그들은 학생들의 토론이 실패했을 때, 부분적으로 그들이 경험이 부족하고 촉진자로서 무엇을 해야 할지 완전히 이해하지 못했기 때문에, "적극적인" 개입을 피하는 경향이 있었다. 또, PBL의 교육 효과에 대해서도 회의적인 견해를 가지고 있어, 지금까지의 교습 경험이나 일본어의 불확실성 회피의 문화적 특징에 의해서도 영향을 받을 가능성이 있다. 하지만, 그들이 경험함에 따라, 그들은 새로운 교육적 맥락으로 사회화되었고, 튜터로서 학생들의 학습을 촉진하는 더 나은 방법을 찾으려고 시도했다. 
In the educational paradigm shift in Japan, the “teacher” is expected to be a facilitator or coach for student learning. Saiki et al. (2013) suggested that Japanese medical educators were sometimes struggling to assume the role of the tutor/facilitator in the PBL tutorial. They tended to avoid “active” involvement when students’ discussion failed, partially because they were inexperienced and did not fully understand what to do as a facilitator. They also had a skeptical view of the educational effectiveness of PBL, which could be affected by their previous teaching/learning experiences and by a cultural characteristic of uncertainty avoidance in Japanese (Saiki et al. 2013). However, as they experienced, they were socialized into the new educational context and attempted to find a better way to facilitate student learning as a tutor.

교육 철학에 대한 몇 가지 공통된 이해는 아시아와 서양 문화 사이에 존재한다. 예를 들면, 일본의 반영과 같은 개념은 「한성hansei」입니다. 이는 "과거 행동을 검토하고, 평가하고, 비판하고, 최종적으로 이를 개선하는 것"으로 정의된다(Taylor et al. 2005). [한세이]는 전통적으로 아이의 사회적, 개인적 발달을 촉진하는 기본 기술로 여겨져 일본 아이들이 한세이의 습관을 들일 수 있도록 장려되어 왔다. 하지만 한세이는 사람이 해서는 안 되는 것, 즉 경험의 부정적인 면을 특히 더 강조하기 때문에, 일본인은 개선을 위하여 실수로부터 배우는 것을 의미하는 「카이젠Kaizen」을 더 선호한다. 이러한 경미한 인식의 차이에도 불구하고, 일본의 의학교육에서는, 지역 의료 사무원(미야타, 야기타 2010)의 포트폴리오나 시뮬레이션에 의한 교육의 보고 등, 「서양」의 성찰 개념이 널리 받아들여지고 있다.

Some shared understandings of educational philosophy do exist amongst Asian and Western cultures (Nishigori and Sriruksa 2011). For example, a concept similar to reflection in Japan is “hansei”. It is defined as “reviewing past behavior, evaluating, critiquing, and finally improving upon it” (Taylor et al. 2005). Hansei has been traditionally considered a fundamental skill that promotes a child’s social and personal development so that Japanese children are encouraged to make a habit of hansei. Hansei emphasizes particularly on the negative aspect of an experience, i.e. what people should not do, so that Japanese people prefer to learn from a mistake for better improvement; this is known as “kaizen” (change for the better) (Kato and Smalley 2010). Despite such minor difference of their perception, the “Western” concept of reflection has been widely accepted in Japanese medical education, such as portfolios in community-medicine clerkships (Miyata and Yagita 2010) and debriefing in simulation-based education (Konishi et al. 2015).

왓킨스와 빅스(1996)는 학습자가 교육적 맥락, 이전의 학습 경험 및 학습 개념에 따라 학습에 다른 접근법을 취할 것이라고 주장했다. 예를 들면, 일본인을 포함한 아시아계 학생은, 「기억」과 「이해」의 관계를 서양의 학생과는 다르게 인식했다. 특히, 아시아 학생들은 암기와 이해를 별개의 과정이라기 보다는, [상호 연결된 과정]으로 보고 암기가 초기 학습 과정에서 이해로 이어질 수 있다고 믿는다. 예를 들면, 학습자가 학습 전에 독학 준비를 하도록 장려하는 뒤집힌 교실(일본어로는 요슈)이 받아들여져 일본의 학부 의학 교육에 서서히 도입되고 있다(Nishiya et al. 2014). 이러한 급속한 보급의 배경에는, 거꾸로 된 교실에서의 학습 과정이, 일본인의 학습에의 어프로치와 일치할 가능성이 있기 때문일 것이다.
Watkins and Biggs (1996) argued that learners would take different approaches to learning in accordance with its educational contexts, their previous learning experiences and conceptions of learning. For instance, Asian students, including Japanese, perceived the relationship between “memorizing” and “understanding” differently than Western students did. Specifically, Asian students do not see memorizing and understanding as separate but rather interconnecting processes, and believe that memorization can lead to understanding in the initial learning process (Watkins and Biggs 1996; Marton et al. 1997; Imafuku 2012). For instance, the flipped classroom, in which learners are encouraged to perform self-study preparation before classroom learning (Yoshyu in Japanese) is now embraced and gradually introduced into undergraduate medical education in Japan (Nishiya et al. 2014). The reason behind this rapid spreading out may be that the learning process in flipped classroom might align with an approach to learning by the Japanese.

일본 학습자는 조용하고 비판적이지 않은 정보수집자이며 암기성 학습자라는 고정관념적인 이미지가 여전히 남아 있다. 그러나, 실제로, 그들은 비판적인 평가 없이 교실에서 적극적으로 기부를 하고 다른 사람들의 의견을 받아들이는 것을 항상 꺼리는 것은 아니었다(Tavakol과 Dennick 2010). 예를 들면, 인터랙티브한 교육 환경에서, 일본어를 포함한 아시아 학습자는, 단지 대화중의 침묵을 언어적 해방이라고만 생각하지 않았다. 오히려 침묵은 상충되는 이해에 대처하는 생산적인 자원, 협업적 관행 및 플랫폼으로 간주되었습니다. 이러한 교육학적 맥락에서 새로운 학습 프로세스를 형성하는 것은 어렵다고 생각되는 것 같았지만, PBL의 경험을 쌓아, 그룹 학습의 성공을 위한 논의에 보다 적극적으로 공헌하려고 했다(Imafuku et al. 2014).
There are still stereotypical images of Japanese learners as being quiet, non-critical recipients of information and rote learners. However, in fact, they were not always reluctant to actively make contributions in the classroom and to accept others’ opinions without a critical appraisal (Tavakol and Dennick 2010). For instance, in the interactive educational setting, Asian learners, including Japanese, did not merely regard silence in interaction as verbal disengagement. Rather, silence was seen as a productive resource, collaborative practice and platform of handling conflicting understandings (Nakane 2006; Jin 2012). Although Japanese learners appeared to feel it difficult to shape the new learning process in such pedagogical contexts, they attempted to more actively make contributions to discussions for the successful group learning, as they acquired experience of PBL (Imafuku et al. 2014).

일본의 의학교육의 미시적 관점에서 보면, 학습이나 행동에 대한 일본인의 어프로치를 지나치게 일반화시키는 리스크에 유의할 필요가 있다. 마츠모토(2002)의 주장처럼, 일본인은 보다 집단주의적이라고 하는 고정관념은, 현대 문화에서의 일본인의 정체성과 커뮤니케이션 행동을 명확하게 반영하고 있지 않다.

From the viewpoint of micro level of Japanese medical education, we need to note the risk of over-generalizing the Japanese learners’ approach to learning and behavior. As Matsumoto (2002) claims, the stereotype of Japanese being more collectivistic does not clearly reflect Japanese identity and communication behavior in contemporary culture.

Conclusions

일본의 의학교육은 아직 유동적이지만, 국제적인 동향에 따라 성공적 발전과 개혁을 볼 수 있다. 국제 트렌드에 대한 열린 마음을 가진 번역주의가 일본을 형성하고 있는 반면, 일본 제품의 외국어로의 번역은 극히 한정되어 있다. 영어권 국가에서 주로 창출되는 지식, 교육용어, 의료교육 모델 등이 의료교육의 글로벌 진전에 기여하는 것이 확실하기 때문에 비영어권 국가에서 동등한 협력을 위해 또 다른 견해와 지혜를 만들어 수출해야 한다. 이를 실현하기 위해, 연구를 통해서 지혜를 수출하고, 설계, 개발, 촉진, 평가, 수송하는 능력을 갖춘 일본의 의학 교육자의 적극적인 참여가 기대된다.

Even though medical education in Japan is still in a state of flux, successful development and reformation can be observed in response to international trends. While “translationalism” with an open mind set for international trends has been shaping Japan, translation of Japanese products into foreign languages has been extremely limited. As knowledge, educational terms, and models of medical education, which are mainly generated in the English-speaking countries surely contributes to the global progression of medical education, another view and wisdom should be produced and exported from non-English speaking countries for the equal collaboration. To realize this, active engagement of Japanese medical educators, who equip the capability to design, develop, facilitate, evaluate, and transport as well as export their wisdom through research are expected.

The truth lies somewhere in the middle: Swinging between globalization and regionalization of medical education in Japan

Abstract

Japan is well known as a super-aging society, with a low birth rate, and has been ranked as one of the countries having the highest quality of healthcare system. Japan's society is currently approaching a major turning point with regard to societal and healthcare reforms, which are influenced by international trends and regional needs. Development of Japanese healthcare human resources, including medical students, is now expected to ride the wave of globalization, while resolving regional problems in the training and delivery of healthcare. Terms and global trends in medical education, such as outcome-based education, community-based education, reflective learning, international accreditation of medical education, and professionalization of educators are well translated into the Japanese language and embraced positively among the Japanese medical educators. However, these trends occasionally sit uncomfortably with cultural variations that are often a common approach in Japan; notably, "hansei" (introspection) and "kaizen" (change for the better). In the world facing a new era where people are unsettled between globalism and regionalism, Japan's future mission is to steer a balanced route that recognizes both global and regional influences and produce global health professionals educators.
