A practical framework for remediating unprofessional behavior and for developing professionalism competencies and a professional identity

Introduction


비전문적 의사 행동은 환자-의사 관계뿐만 아니라 환자 안전 및 의료 품질에도 영향을 미친다(Hickson et al. 2002; Rosenstein 2011, 2015). 학부 및 대학원 의과대학에서의 비전문적 행동과 의사가 된 이후에서의 비전문적 행동 사이에 명확한 연관성이 연구 결과 드러나면서 의료실무에서의 전문적 행동에 대한 관심이 탄력을 받게 되었다(파파다키스 외). 2004, 2005, 2008). 이러한 연관성은 가능한 한 일찍 비전문적인 행동을 다루는 것의 중요성을 강조한다.

Unprofessional physician behavior affects patient–doctor relationships as well as patient safety and quality of care (Hickson et al. 2002; Rosenstein 2011, 2015). Interest in professional behavior in medical practice gained momentum when research showed a clear association between unprofessional behavior in undergraduate and postgraduate medical school and unprofessional behavior in later practice (Papadakis et al. 2004, 2005, 2008). This association underscores the importance of addressing unprofessional behavior as early as possible.

Although only a small number of students display unprofessional behavior (Mak-van der Vossen et al. 2016; Barnhoorn et al. 2017), unprofessional behavior can harm patients and their trust in the medical profession (Hickson et al. 2002; Rosenstein 2011, 2015; Ellaway et al. 2017). However, assessment and behavior remediation of unprofessional remain difficult tasks

Sara는 22세의 의대 학생으로, 의대 전문 행동 위원회에 회부되었다. 간단한 소개서에는 "의사가 되기 위한 올바른 역량의 부족"이 언급되어 있다. 담당 의사에게 전화 한 통을 받은 결과, 산부인과에서 일하는 동안 그녀의 상사들이 사라에게 "너무 적게 참여한다"거나 심지어 "일부 동안 어딨는지 알 수 없다"는 피드백을 줬다는 사실이 밝혀졌다. 반복된 피드백에도 불구하고, 사라는 계속 표준 이하로substandardly 출석했는가? 전문 행동 위원회의 두 멤버와의 회의에서, 사라는 왜 주어진 피드백에 따라 행동하지 않았는지 물었다. 그녀는 대답하였다. Sara is a 22-year-old medical student who has been referred to the professional behavior board of her medical school. The brief referral letter mentions a “lack of the right competencies to become a doctor”. A phone call to the referring doctor reveals that her supervisors during the clerkship gynecology have given Sara feedback on “participating too little” or even “being untraceable for parts of the day”. Despite repeated feedback, Sara continued to participate substandardly? In a meeting with two members of the professional behavior board, Sara asked why she did not act upon the given feedback. She answered: “I try to participate in the clerkships wherever I can. But due to my PhD trajectory I have to spend my time efficiently, so I plan to attend only the parts of the clerkship that are relevant for my future career”.



비전문적 행동의 교정조치는 전문성에 대한 서로 다른 관점과 이러한 관점을 연결하는 어려움으로 인해 방해받는다(Birden et al. 2014; Irby and Hamstra 2016; Smith et al. 2017). 최근까지 전문성 담론에서는 두 가지 관점이 두드러졌다.

The remediation of unprofessional behavior is hampered by the different perspectives on professionalism and by difficulties in bridging these perspectives (Birden et al. 2014; Irby and Hamstra 2016; Smith et al. 2017). Until recently, two perspectives were prominent in the professionalism discourse.

On the one hand, we find a pragmatic, but narrow focus on professional behavior. The focus on behavior, however, has as the advantage that it can be observed and assessed and expectations for desirable behavior can be set relatively easily (Barnhoorn and van Mook 2015).

On the other hand, there is a focus on professionalism, referring to character traits, attitudes, virtues and values (van Mook, de Grave, et al. 2009; van Mook, van Luijk, et al. 2009; Barnhoorn and van Mook 2015). This second focus helps to motivate and inspire students, but it is difficult to assess (Barnhoorn and van Mook 2015; Irby and Hamstra 2016).

Both perspectives capture important aspects of the process of becoming a professional physician, but what is missing in both is a broader view on the process of developing into a professional (Holden et al. 2015). The focus, therefore, shifted to the perspective of “Professional Identity Formation” (PIF). PIF can be defined as the process of forming “a representation of self, achieved in stages over time during which the characteristics, values, and norms of the medical profession are internalized, resulting in an individual thinking, acting and feeling like a physician” (Cruess et al. 2014).

Several authors have proposed frameworks to address unprofessional behavior that combine elements from the different perspectives


프레임워크는 행동, 태도, 프로세스 요소를 기술하지만, 이들 요소 사이의 상호관계를 탐구하는 것은 없다.

The frameworks describe behavior, attitude, and process elements, but none explores the interrelationship between these elements.


행동, 태도 또는 과정 중 어느 한쪽의 관점을 넘어 비전문적인 행동을 논하기 위해서는 종합적인 프레임워크가 필요하다(Boileau et al. 2017). 이런 틀의 부재는 교사들에게 비전문적인 행동을 효과적으로 다루기엔 역부족이라는 인식을 심어준다.

A comprehensive framework is needed to discuss unprofessional behavior beyond the perspective of either behavior, attitude, or process (Boileau et al. 2017). The absence of such a framework feeds the perception of teachers that they are ill-equipped to address unprofessional behavior effectively.


다수준 프로페셔널리즘 프레임워크

The multi-level professionalism framework


다단계 전문성 프레임워크가 성찰과 발전의 틀이 될 수 있는 만큼 다양한 직업, 특히 의료계, 의료교육에 있어 가치가 있을 것으로 기대한다. 코타겐은 환경, 행동, 역량, 신념, 정체성, 그리고 모델의 중심에서 미션(코타겐 2004)의 6단계로 구성된 모델을 개발했다. 이 모델은 좋은 교사의 본질적인 자질이 행동이나 역량의 측면에서만 설명되어야 하는 것이 아니라, 더 많은 측면을 고려해야 한다는 것을 보여준다. 예를 들어, 역량은 행동의 잠재력으로 볼 수 있지만, 어떤 역량이 실행에 옮겨질 것인지는, 어떻게 옮겨질 것인지는 환경에 의존적이다 (Kortagen 2004).

We expect the multi-level professionalism framework to be of value for various professions, in particular for the medical profession, and medical education, as it can serve as a framework for reflection and development. Korthagen developed a model consisting of six levels: environment, behavior, competencies, beliefs, identity, and, at the model’s center, mission (Korthagen 2004). The model shows that the essential qualities of a good teacher should not only be described in terms of behavior or competencies; more aspects should be taken into account. Competencies, for example can be seen as potential for behavior, but which competencies will be put into practice and how depends on the environment (Korthagen 2004).

환경: 여기가 어디지?

Environment: Where am I?


환경 수준이란 의대생이 생활하고 일하고 배우는 다양한 맥락과 그 행동에 영향을 미치는 상황을 말한다. 의학 교육의 영역 내에서, 그것은 다음의 두 가지이다.

[학술적academic 학습 환경] (숨겨진 커리큘럼과 공식적인 커리큘럼), 그리고 

[임상적 환자 관련 환경]이다(Hafferty 1998; Franco et al. 2017; Hawick et al. 2017). 

[학생의 개인 환경]은 가정에서의 문제, 신체적 또는 정신 질환, 개인 이력 등 자신의 사생활과 상황을 가리킨다(Mak-van der Vossen et al. 2016). 이것들은 학생의 행동과 발전에 영향을 미친다. 

The environment level refers to the diverse contexts in which the medical student lives, works and learns, and which influence his or her behavior. Within the realm of medical education, it describes the academic learning environment: both the hidden and the formal curriculum, as well as the clinical, patient-related environment (Hafferty 1998; Franco et al. 2017; Hawick et al. 2017). The personal environment of the student refers to his or her personal life and circumstances, such as problems at home, physical or mental illness and personal history (Mak-van der Vossen et al. 2016). These influence the student’s behavior and development. 


따라서, 환경은 또한 좁든 넓든 방식으로 학생의 전문적인 수행에 지대한 영향을 미칠 수 있다. 학생의 행동은 상황, 즉 환경이 해석에 포함될 때만 이해할 수 있다(Kortagen 2004; Jha et al. 2016; Franco et al. 2017; Mason 2017).

Therefore, the environment can also have a profound influence on a student’s professional performance, both in a limiting and an expanding way. Student behavior can only be understood when the context, i.e. the environment is included in the interpretation (Korthagen 2004; Jha et al. 2016; Franco et al. 2017; Mason 2017).

Behavior: What am I doing?


행동 수준의 초점은 [학생의 퍼포먼스]로, 직접 관찰하고 평가할 수 있다. 실제 환자와의 만남은 물론 학생과 교직원, 학생과 또래와의 만남이 이뤄지는 것도 이 정도 수준이기 때문에 행동이 많은 관심을 끈다. 환경과는 별개로 행동이란 [(프레임워크의 내면에 표현되어 있는) 사람 내부의 다양한 힘]에 의해 동기가 부여된다.

The focal point at the behavioral level is the performance of the student, which can be directly observed and assessed. Behavior attracts much attention, because it is at this level that actual patient encounters take place, as well as encounters between students and faculty, and students and peers. Apart from the environment, behavior is motivated by various forces within the person, which are represented in the inner circles of the framework.


역량: 내가 무엇을 할 수 있을까?

Competencies: What can I do?


역량은 전문적 행동을 가능하게 하는 지식, 기술 및 태도의 통합체로 정의할 수 있다(Stoof et al. 2002). 이들은 의료 교육 분야에서 확고한 기반을 얻었다(Hunderfund et al. 2017). 마찬가지로 전문성 프로그램도 역량 측면에서 편성되는 경우가 많다(Royal College of Occessors and Occessors in Canada 2017). 역량은 행동과 밀접하게 연관되어 있으며 행동의 잠재력을 나타낸다. 그러나 역량이 실행에 옮겨질지는 무엇보다도 상황이나 환경, 그리고 기초적인 신념, 가치관, 정체성, 미션에 달려 있다. 학생들은 자신의 행동을 당면한 상황에 맞출 수 있도록 자신의 역량을 언제, 어떻게 실천에 옮겨야 하는지에 대한 성찰이 필요하다.

Competencies can be defined as an integrated body of knowledge, skills, and attitudes (Stoof et al. 2002) that make professional behavior possible. They have gained a strong basis in medical education (Hunderfund et al. 2017). Likewise, professionalism programs are often formulated in terms of competencies (Royal College of Physicians and Surgeons in Canada 2017). Competencies are closely connected to behavior, they represent a potential for behavior. However, whether competencies are put into practice depends, the among other things, on circumstances or environment, and on underlying beliefs, values, identity, and mission. Students need to reflect on when and how to put their competencies into practice, in order to be able to match their behavior to the situation at hand.

믿음과 가치관의 수준은 의대생이 [의료전문직]과 [그 안에서의 자신의 위치]와 관련하여 가지고 있는 개념과 신념을 다룬다. 예를 들어, 학생들은 [질병에 초점을 맞춘 신념]이나 [좀더 환자 중심적인 믿음]을 가질 수 있다. 학생들이 가지고 있는 신념은 특정 역량을 개발하려는 학생의 동기에 영향을 미친다

    • 의학전문가의 핵심 역할이 질병 치유라고 믿는 학생이 있다면 기술역량 개발에 집중하는 경향이 있을 것이다. 

    • 학생이 대신 환자를 돌보는 것이 의학의 중심이라고 믿는다면 의사소통 역량을 키워야 할 강력한 동기를 기대할 수 있다. 

The level of beliefs and values deals with the conceptions and convictions a medical student holds true regarding the medical profession and his or her place in it. For instance, students can adopt a disease-focused belief or a more patient-focused belief. The beliefs students hold influence a student’s motivation to develop certain competencies: 

    • If a student believes that the main role of the medical professional to be curing disease, there will be a tendency to focus on developing technical competencies. 

    • If a student instead believes caring for patients is central to medicine, a strong motivation to develop communication competencies can be expected. 


하지만, 반대로 행동과 역량은 학생들의 믿음에 영향을 미친다. 예를 들어, 의사소통 능력을 개발하는 것은 학생의 면접 방식을 더 효과적일 뿐만 아니라, 의사소통에 대한 기본적인 믿음을 바꿀 수도 있다. 예를 들어, 따뜻하고 공감하는 것뿐만 아니라, 어떤 경우에는 커뮤니케이션이 약을 처방하거나 수술을 하는 것보다 더 효과적일 수 있다.(Jenkins and Fallowfield 2002).

However, also the other way around, behavior and competencies influence student’s beliefs. For example developing communication competencies does not only make the student’s interviewing style more effective, it may also alter underlying beliefs about communication, for instance that it is not only about being warm and empathetic, but also that in some instances in medical care communication may be more effective than prescribing pills or operating (Jenkins and Fallowfield 2002).

정체성은 [직업의 특성, 가치관, 규범을 포함하여 특성, 가치관, 규범 측면에서 자신을 정의하는 방식]이라고 설명할 수 있다(Kortagen 2004; Cruess et al. 2014). 예를 들어 '나는 누구인가', '나는 어떤 의사가 되고 싶은가' 등의 질문에 성찰을 하면서 자신에 대해 더 많이 배울수록 자신의 정체성이 행동에 어떤 영향을 미치는지 더 잘 알게 된다. 정체성 평가에 대한 상세한 가이드는 베보, 파버랑겐덴(2014년)이 제공했다.

Identity can be described as the way one defines oneself in terms of characteristics, values, and norms, including the characteristics, values, and norms of the profession (Korthagen 2004; Cruess et al. 2014). The more students learn about themselves, for example by reflecting on questions such as “who am I?” and “what kind of doctor do I want to be?”, the more they are aware of how their identity influences their behavior. A detailed guide for the assessment of identity has been provided by Bebeau and Faber-Langendoen (2014).


사명: 왜 내가 하는 일을 하지?

Mission: Why do I do what I do?


여기서 문제는 무엇이 한 학생으로 하여금 지금 하는 일을 하게 만드느냐는 것이. 사명 레벨은 '의사가 되고 싶은 목적이 무엇인가' '의사로서 나의 개인적인 소명은 무엇인가' 등 고도의 개인적인 질문에 대한 대답에 관한 것이다. 코스타겐에 따르면, 이 수준은 사람들의 원동력driving force을 묘사한다; 우리가 지금 하는 일을 하도록 움직이게 하는 것은 내면 깊은 곳의 무엇일까. 그것은 더 큰 전체 안에서 자신의 존재에 의미를 부여한다(Kortagen 2004). 미션과 정체성의 차이점은, 정체성이 의대생의 자아상self-image에 대한 것이라면, 미션은 의대생이 다른 사람과의 관계에 있어서 자신의 역할을 무엇으로 보는가에 관한 것이다.

The question here is what moves a student to do what he or she does. The mission level is about answers to highly personal questions, such as “to what end do I want to become a doctor?” and “what is my personal calling as a doctor?”. According to Korthagen, this level describes people’s driving force; what is it deep inside us that move to do what we do. It gives meaning to one’s own existence within a larger whole (Korthagen 2004). The mission level differs from the identity level in that refers to medical students’ self-image, whereas the mission level is about what role the medical student sees for himor her-self in relation to others.

이제 우리는 다단계 전문성 프레임워크가 어떻게 의료 교육자들이 학생들을 전문적 행동의 발달에 지원하도록 도울 수 있는지 알아보기 위해 이 사례를 다시 살펴본다.

We now revisit the case in order to explore how the multilevel professionalism framework can help medical educators to support students in developing professional behavior.

The case revisited 1 


새라의 행동(참여 부족)은 선생님들에게 인정받지 못했다. 그녀가 미래의 의사를 위한 올바른 능력이 부족하다고 말함으로써, referring doctor는 "미래 의사를 위한 올바른 능력"과 같은 것이 있다는 것을 암시한다. 그러나 사라는 자신이 어디로 가고 있는지 이미 알고 있기 때문에 임상실습의 모든 부분에 참여할 필요는 없다고 생각한다.

Sara’s behavior (her lack of participation) was not appreciated by her teachers. By stating that she lacks the right competencies for a future doctor, the referring doctor implies there is such thing as the “right competencies for a future doctor”. Sara, however, believes she does not need to participate in every part of the clerkship, knowing already where she is heading.

사건 재조사 2 

The case revisited 2 


위원회는 우선 비전문가로 낙인찍힌 행동에 대해 사라와 합의를 볼 필요가 있었다. 사라는 대부분의 임상실습중 많은 부분이 그녀의 흥미를 끌지 못한다는 것과 그녀가 놓칠 수 있을 때 안과에서 박사학위 궤적을 연구한다는 것에 동의한다. 

  • 환경과 관련하여, 사라씨는 몇몇 전문가들이 임상 전 기간 동안의 강의에서 최고점에 도달하기 위해 초점의 중요성을 강조했다고 말한다. 

  • 그녀는 "만약 목표가 있다면, 그것을 고수하라!"라는 자신의 모토를 따라 산다. 게다가, 사라는 자신이 좋은 의사가 되기 위해 필요한 기본적인 능력을 이미 갖추고 있다고 생각한다.

The board first needed to reach consensus with Sara about the behavior that has been labeled as unprofessional. Sara agrees that large parts of most clerkships do not interest her, and that, when she can be missed, she works on her PhD trajectory in ophthalmology. 

  • Concerning the environment, Sara says that several specialists, in their lectures during her pre-clinical years, as well as supervisors during some clerkship stressed the importance of focus in order to reach the top. 

  • She lives by her motto: “If you have a goal, stick to it!” Besides, Sara thinks she already possesses the basic competencies needed to become a good doctor. 

  • She says she shares the belief that most of the clerkships, for her, are redundant. After all, she already knows what she wants to become and believes that she better spend her time on her PhD trajectory. 

  • Ultimately, the board wanted to talk to Sara about her professional identity. Sara’s answer to the question “what kind of doctor do you want to become?” is that she wants to reach the absolute top in ophthalmology. 

  • At the level of mission, when asked “why do you want to become a top ophthalmologist?”, Sara states that the societal status of a superspecialist appeals to her and that the practical and technical aspects of ophthalmology are the main reasons for her preference.

Discussion


최근 의학교육 목표를 전문직업성을 가르치고 평가하는 것에서 전문직 정체성 형성으로 바꾸자는 제안도 비전문적 행동이 어떻게 다뤄질지 시사하는 바가 있다(Cruess et al. 2014). 비전문가적 행동의 교정조치는 다르게 접근될 수 있지만 여전히 도전적인 것으로 남을 것이다(Bebeau 및 Faber-Langendoen 2014; Arnold et al. 2016). 행동, 태도 또는 프로세스 기반에 관계없이 프로페셔널리즘에 대한 각각의 일반적인 관점은 비프로페셔널한 행동을 다르게 보고 결과적으로 서로 다른 교정조치 전략을 권고한다(Irby와 Hamstra 2016).

The recent proposal to change the goals of medical education from teaching and assessing professionalism to the formation of a professional identity also has implications for how unprofessional behavior will be addressed (Cruess et al. 2014). Remediation of unprofessional behavior can be approached differently, but will remain challenging (Bebeau and Faber-Langendoen 2014; Arnold et al. 2016). Each of the prevailing perspectives on professionalism, whether behavior-, attitude-, or process-based, views unprofessional behavior differently, and consequently, different remediation strategies are recommended (Irby and Hamstra 2016).


[행동 기반 관점]에서 보면, 의학교육의 목표는 [전문적으로 행동하는, 사회에 유능한 의사]를 전달하는 것이다. 행동은 측정할 수 있는 것으로 보이며, 의대생의 행동이 비전문적이라고 판단되면 교정조치가 적용된다. 여기에는 종종 행동 식별, 행동 논의, 교정조치 계획 수립, 교정조치 계획 준수 여부 모니터링이 포함되며 때로는 보호관찰 또는 학생의 해고를 포함한다(Domen 2014; Irby and Hamstra 2016; Banhoorn et al. 2017).

From the behavior-based perspective, the goal of medical education is to deliver competent doctors to society who behaves professionally. Behavior is seen as measurable, and if behavior of a medical student is judged as unprofessional, remediation is applied. This often involves identifying behavior, discussing behavior, developing a remediation plan, monitoring compliance to the remediation plan and sometimes includes probation, or the dismissal of the student (Domen 2014; Irby and Hamstra 2016; Barnhoorn et al. 2017).


[태도 기반 관점]에서 의료 교육의 목표는 좋은 의료 실천에 필요한 덕목과 가치를 기르는 것이다. 미덕과 가치는 측정하기 어렵다. 프로페셔널하지 못한 행동의 경우 교정조치는 성찰 과제와 피드백을 포함한다(Irby and Hamstra 2016).

From the attitude-based perspective, the goal of medical education is fostering the virtues and values needed for good medical practice. Virtues and values are difficult to measure. In case of unprofessional behavior, remediation involves reflection assignments and feedback (Irby and Hamstra 2016).


[프로세스 기반 관점]에서 의료 교육의 목표는 전문적 정체성의 형성이다(Cruess et al. 2014). 이러한 전문적 정체성은 시간 경과에 따라 단계적으로 달성된다(Cruess et al. 2014). 프로답지 못한 행동은 다음 발달 단계로 나아가는 데 실패한 것으로 볼 수 있다. 이러한 관점에 따르면 교정조치는 발달 단계에 맞게 조정되어야 하며, 학생들이 "그들이 누구인지, 그들이 누가 되고 싶은지 알아내기 위해" 지속적이고 오랜 시간에 걸쳐 도움을 필요로 한다(Cruess et al. 2014).

From the process-based perspective, a goal of medical education is the formation of a professional identity (Cruess et al. 2014). This professional identity is achieved over time, in stages (Cruess et al. 2014). Unprofessional behavior can be seen as a failure to progress to the next developmental stage. According to this perspective, remediation must be tailored to the developmental stages, and involves helping students, continuously and over a long period of time, “to find out who they are, who they are becoming and who they wish to become” (Cruess et al. 2014).

The multi-level professionalism framework, however, is not unique in linking different perspectives on professionalism. Cruess et al. have linked the behavior-based perspective to the PIF-perspective in Miller’s “Amended Pyramid” (Cruess, Cruess, Steinert, et al. 2015). In addition to the original pyramid, they propose an extra level of “thinking, acting and feeling like a physician” (“Is”) at the apex of the pyramid (Cruess, Cruess, Steinert, et al. 2015). Presented as an assessment tool for professional identity, they suggest that the model helps in creating a perspective on assessment of PIF (Cruess et al. 2016). However, it does not provide tools for educators to address unprofessional behavior. The proposed multi-level professionalism framework, on the other hand, describes the nonhierarchical interplay between the different levels relevant to professionalism.

The multi-level professionalism framework offers a practical guidance for the remediation which bridges the various perspectives on professionalism.


다단계 프로페셔널리즘 틀은 프로페셔널리즘의 청사진을 나타내는 척도 하지 않고 프로페셔널리즘의 '본질'을 포착하는 척도 하지 않는다. 프로페셔널리즘에서 중요한 관점을 다루면서, 그것은 학생들과 교사들에게 행동의 수준 그 이상의 것에 대해 성찰하도록 한다. 프레임워크로 작업하는 것은 더 깊은 학습이 이루어질 수 있음을 시사한다.

The multi-level professionalism framework neither pretends to denote a blueprint for professionalism, nor pretends to capture the “essence” of professionalism. Addressing the important perspectives in professionalism, it invites students and teachers to reflect on more than just the level of behavior itself. Working with the framework suggests that deeper learning can be attained.

A practical framework for remediating unprofessional behavior and for developing professionalism competencies and a professional identity

Abstract

The relatively new term "Professional Identity Formation" (PIF) complements behavior-based and attitude-based perspectives on professionalism. Unprofessional behavior and its remediation should also be addressed from this perspective. However, a framework is needed to guide discussion and remediation of unprofessional behavior, which can encompass behavior-based, attitude-based, and identity-based perspectives on professionalism. To this end, the authors propose a multi-level professionalism framework which describes, apart from professional behavior, more levels which influence professional performance: environment, competencies, beliefs, values, identity, and mission. The different levels can provide tools for educators to address and discuss unprofessional behavior with their students in a comprehensive way. By reflecting on all the different levels of the framework, educators guard themselves against narrowing the discussion to either professional behavior or professional identity. The multi-level professionalism framework can help educators and students to gain a better understanding of the root of unprofessional behavior, and of remediation strategies that would be appropriate. For despite the recent emphasis on PIF, unprofessional behavior and its remediation will remain important issues in medical education.
